**About the Opportunity**

Reporting to the Manager Healthy Communities, the Youth Health Centre Coordinator (YHCC) is an active member of the Healthy Communities Public Health Team and works within the Public Health Standards and Protocols and a Health Promoting Schools Approach, guided by the Public Health, Health Promoting Schools Standards.

The YHCC provides leadership, informal assessment, service navigation, youth engagement, and support for youth associated with the assigned school facility and its immediate community. The YHCC champions youth health, and health equity and facilitates youth engagement as a component of health-promoting schools. The position works with youth and fosters cooperation and collaboration among school administrators, teachers, SchoolsPlus, guidance, student/youth support workers, primary health care, mental health and addictions, and other community partners to ensure youth have access to support, programs, and services within the school and community that promote health. The YHCC models and provides safe, equitable, and inclusive youth support and understands the diverse experiences of the school and the surrounding community.

Relevant du directeur des Communautés en santé, le coordonnateur ou la coordonnatrice de centre de santé jeunesse fait partie de l’équipe de santé publique des Communautés en santé et travaille dans le cadre des normes et protocoles de santé publique ainsi que de la stratégie générale liée aux écoles-santé, étant guidé-e par les normes de la Santé publique et celles des écoles-santé.

Le coordonnateur ou la coordonnatrice de centre de santé jeunesse offre un leadership, fait des évaluations non officielles, aide à s’y retrouver dans les services, favorise l’engagement des jeunes et soutient les jeunes des écoles qui lui sont affectées ainsi que pour la communauté immédiate. Il/elle défend la santé des jeunes et l’équité en matière de santé, ainsi que la participation des jeunes comme acteurs au sein des écoles-santé.

Le coordonnateur ou la coordonnatrice de centre de santé jeunesse intervient donc auprès de jeunes et favorise la coopération et la collaboration entre les administrateurs scolaires, les enseignants, ÉcolesPlus, les services d’orientation, les travailleurs qui viennent en aide aux jeunes et aux élèves, les services de santé primaires, les services de santé mentale et de traitement des dépendances, ainsi que d’autres partenaires communautaires, afin que les jeunes aient accès, dans les écoles et la collectivité, à des soutiens, programmes et services de promotion de la santé. Le coordonnateur ou la coordonnatrice du Centre de santé jeunesse offre aux jeunes un soutien sûr, équitable et inclusif et comprend la diversité d’expériences des jeunes aussi bien à l’école qu’à l’extérieur.

**About You**

We would love to hear from you if you have the following:

- Baccalaureate degree in a relevant health care discipline (registered nurse, occupational therapist, social work, nutritionist).
- Minimum of three years’ experience in a relevant field; directly working with youth required
- Registered/licensed with your regulated health care discipline, college or association is required (registered nurse, occupational therapist, social work, nutritionist).
- Must be bilingual in French and English
- Must have a valid driver’s license and access to a reliable vehicle
- Ability to work independently and as a member of a multidisciplinary team both within the organization and in the community
- Ability to critically assess situations and navigate complex scenarios using sound decision-making and a non-judgmental, trauma-informed approach in supporting youth.
- Demonstrated ability and experience in creating opportunities for meaningful youth engagement and fostering youth leadership.
- Demonstrated understanding of the social determinants of health, health equity
