**Responsibilities**:

- Provide Consultation, recommendations and interventions to clients in their home using evidence based best practice.
- Assess psychosocial needs of clients and their families in response to illness/changing care needs and their related ability to adjust and/or cope.
- Counsel clients and their family members concerning identified problems.
- Counsel clients and their family members concerning long range planning.
- Provide short-term therapy to clients and family members.
- Problem solve in collaboration with client and care team to create opportunities for more independence.

**Ed**ucation and Experien**ce**:

- Master of Social Work Degree from a recognized university.
- In good standing with the Ontario College of Social workers and Social Service Workers.
- Minimum 2 years of Clinical experience.
- Valid driver's license with access to a vehicle.

Most of our rehabilitation services are provided free of charge through our contract with the Champlain Home and Community Care Support Services (HCCSS) and the Ottawa and Ottawa East Ontario Health Teams.
