**What We Will Ask of You**: As a key member of our Adult Urgent Response Services (AUR) team, our Crisis Response Network Coordinator (CRNC) plays an important role. On most days you’ll be coordinating short term specialized services for adults who live with a developmental disability, mental health need, challenging behaviour and/or involved or at risk of involvement within the criminal justice system as deemed eligible by Developmental Services Ontario (DSO). In collaboration with the Team Lead and AUR team, the CRNC conducts risk assessments and ensures adequate supports are in place to facilitate a crisis resolution in line with best practice for quality service provision. The CRNC is a champion of Canopy’s core value of Collaboration who develops community connections, navigates sector resources and grows partnerships for ongoing supports.

**Primary Responsibilities**:

- Collaborate with Team Lead on received DSO referrals and provide service consultation to adults living with a developmental disability and/or mental health need who are experiencing crisis as deemed eligible through MCCSS.
- Provide referral(s) to Developmental Services Ontario (DSO) as directed for individual’s requiring additional services within Individual’s Support Plan (ISP).
- Collaborate with Canopy staff and other external partners regarding complex support needs.
- Complete funding requests for individuals and manage approved funds, including reviewing invoices and tracking in collaboration with the HKPR partnering agencies.
- Attend applicable case conferences, local and regional committees and other meetings to foster and grow crisis response partnership and case resolution initiatives for individuals accessing services.
- Provide guidance and expertise of sector resources to individuals navigating service options including; Adult Service Coordination Committee, Developmental Services Ontario (DSO) and Canopy.
- In conjunction with the Program Manager and Team Lead, provide ongoing support and collaboration in developing community outreach materials including; Education events, communicating information sessions and supporting with other event related activities.

**Qualifications**:

- Completion of Bachelor’s or Master’s degree in Social Sciences or related field or equivalent work experience.
- 3-5 years of experience in Case Management or short-term consultation experience in a residential or community setting.
- Case Management or mental health experience is considered an asset.
- Direct experience working with people with an intellectual disability, challenging behaviours, dual diagnosis or mental health need or equivalent.
- Valid G-Class driver’s license for the Province of Ontario, access to reliable transportation and have valid proof of automobile insurance with driving for business purposes and a minimum liability of two million dollars.
