**Jordan’s Principle Outreach Worker**

Zagimē Anishinabēk is a progressive First Nations government, located around 22 KM Northeast of Grenfell Town in Saskatchewan.

**SUMMARY**:Jordan’s Principle Outreach Worker for Zagime Anishinabek is mandated to increase awareness, access, and case management on behalf of the Jordan’s Principle Program for the children/families with complex or unmet medical needs.

**DUTIES**:
The Outreach Worker would function within a multi-disciplinary, coordinated service approach within the following areas:
1. Case Management
- Provide a collaborative, client driven system to guide children and their families through a comprehensive array of services
- Support a child and family
- centered approach to coordinating and connecting services
- Work towards establishing multi-disciplinary hubs for families when the services they require involves several professionals

2. Intake, Assessment and Coordination
- Help families navigate the health and social systems in a timely manner
- Help with referrals for assessments to appropriate professional, or provide assessment if within scope of practice
- Build case management capacity through the development of policies, procedures and protocols, related to screening and assessment, service planning, monitoring and evaluation of service plans and staff training

3. Outreach and Community Engagement
- Reach out to Zagime Anishinabek families and invite them to use the enhanced service coordination
- Enhance awareness in communities of existing services and supports
- Build relationships with the community and collaborate with those providing existing community-based services
- Work to proactively identify children with unmet needs to facilitate early intervention and timely access to services and resources.

**QUALIFICATIONS**:

- Post-secondary education in the Health, Social Sciences, or Education sector; or an equivalent combination of education and experience will be considered.
- Work experience in a service collaboration environment and client centered services delivery model.
- Demonstrate leadership ability in chairing meetings, team building, and reporting.
- Strong verbal and written communication skills.
- Submit a Criminal Record Check and Pre-employment Substance Test prior to commencing employment.
- Valid Driver’s License and a reliable vehicle would be an asset.
