Department: Resident Services

Position: Admissions Coordinator

Weekdays 9:00am-5:00pm (75 hours a bi-weekly)

Weekends and evening work may be required.

Status: Temporary Full Time

Date Required: ASAP

Reporting to the Director, Residents Services the Quality and Digital Health Lead is responsible Coordinate and facilitate all aspects of Admissions, internal transfers and discharges within the organization.

EXPERIENCE & EDUCATION
- Diploma in Social Services - BSW preferred.
- Bilingual - English and Italian
- Diploma in Geriatrics or 2 years’ experience in a similar field.
- Excellent communication and organizational skills.
- Strong prioritization and organizational skills required to meet the demands of a dual role.

- Demonstrates skill in managing aggressive behaviour using client centered approach.
- Ability to speak Italian an asset.

RESPONSIBILITIES INCLUDE (but are not limited to):

- Reviewing all new updated information focusing on the medical and psychosocial aspect of the client.
- Requesting additional information from HCCSS regarding applicant.
- Chairing the resident admissions and transfer committee weekly.
- Answering all community inquiries regarding admissions to Villa Colombo.
- Conducting weekly tours with possible applicants and family members.
- Providing support and counselling to family members and clients (usually occurs on the day of admissions or when conducting tours).
- Coordinating multidisciplinary team members for admission assessments.
- Scheduling and organizing the Short Stay program.
- Completing admissions contract with family/client on day of admission.
- Preparing monthly utilization statistics.
- Other related duties as required.
