SHIP is presently looking for a **Community Mental Health Counsellor** who will provide treatment and support with system transitions to clients experiencing mental health challenges and addictions in the Dufferin community.

**Duties and Responsibilities**
- Formulate an implement client-centered service plans
- Provide time limited, short term, case management
- Review referrals and conduct clinical assessments
- Provide education on mental illness and crisis management
- Develop follow-up plans as necessary
- Connect clients to longer term service, if required
- Refer to partnering organizations and facilitate linkages to community supports
- Provide streamlined transitions with “warm transfers”
- Partner with clients and their families as members of the health care team
- Liaise with community supports (i.e. mental health and addiction service providers, and family members) as required

**Qualifications**
- Post-secondary education in Social Work, Health Services, or equivalent
- 3+ years experience supporting individuals with mental health challenges in the community
- Any experience providing DBT treatment is an asset
- Certification with the Ontario College of Social Workers is an asset
- Must have valid drivers license and access to a reliable vehicle

**Working Conditions and Physical Requirements**
- Day shifts (Monday - Friday) with the ability to work excess/overtime hours where required
- Travel by vehicle to community sites for department/program support as required
