**Responsibilities**:

- Provide support and guidance to clients.
- Conduct assessments of clients’ needs and develop individualized service plans.
- Maintain accurate records of client information and progress.
- Conduct home visits and provide in-home support services.
- Collaborate with other agencies to coordinate services for clients.
- Provide crisis intervention and advocacy services.
- Assist clients in accessing community resources.
- Develop and maintain relationships with clients.

**Job Types**: Full-time, Part-time, Permanent, Fixed term contract
Contract length: 6 months
Part-time hours: 40 per week

**Salary**: $18.00-$20.00 per hour

- French not required

Schedule:

- 8 hour shift
- Day shift
- Monday to Friday
- Weekend availability

Supplemental pay types:

- Bonus pay
- Commission pay
- Overtime pay

Ability to commute/relocate:

- Brampton, ON L6W 3W8: reliably commute or plan to relocate before starting work (required)

**Education**:

- Secondary School (preferred)
