**HOURS: ALL SHIFTS AS REQUIRED
**QUALIFICATIONS INCLUDE: **

Minimum of a four (4) year Bachelor’s degree in a health related field, from an accredited university. 
Current certification of Registration in good standing as a Health Professional with an Ontario Professional

Associations or regulatory college. 

Minimum 5 years relevant experience working within a health care setting with a focus on older adults. 
Demonstrated person and caregiver focused care and supported by best evidence-based practices within

respective scope of practice. 

Applied knowledge of health assessment tools (e.g. Comprehensive Geriatric Assessment (CGA), Barthel
Index, Confusion Assessment Method (CAM), Mini Mental State Examination (MMSE)). 

Demonstrated skills conducting comprehensive geriatric assessment, analyzing, developing and administering
treatment plans based on the comprehensive geriatric assessment. 

Demonstrated knowledge of Specialized Geriatric Services across the continuum including primary care and
community support services (i.e. Alzheimer’s society, Home and Community care, etc.). 

frameworks and choose the optimal assessment and treatment intervention. 

Demonstrated knowledge of best practices for dementia, delirium, depression and other mental health issues
and their effect on patients, families and service providers. 

Demonstrated skills in management of geriatric syndromes, including delirium, polypharmacy, falls and
functional decline, acute/sub-acute cognitive changes, bowel/bladder, and nutrition/hydration problems. 

Demonstrated strong therapeutic communication skills and ability to communicate both verbally and in writing in
a clear, concise manner, considering the intended audience. 

Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and
rapidly changing priorities and challenging deadlines. 

Ability to participate as a member of a multidisciplinary team with the ability to function well in a team oriented
climate required.** **

Must demonstrate commitment to ensure their safety, the safety of fellow workers, patients, volunteers,
and visitors.** **

Must support and contribute to a culture of safety and prevention of adverse health events in this organization.** **
Good attendance and work record required.** **
