The BSO Registered Practical Nurse (RPN) is clinical support for the Chatham Kent BSO Team supporting older adults experiencing responsive behaviours associated with dementia, mental health, addictions and other neurological conditions and their care partners.

The BSO RPN provides clinical assistance to the team members of the BSO LTC Lead Team of Chatham-Kent. The RPN will coordinate all activities and services provided by the BSO LTC Lead Team for the LTC homes in the Chatham-Kent area. The RPN works across health care sectors in collaboration with BSO Supports, including: the BSO Long-term Care Lead Teams and Specialized Social Therapists, BSO Hospital Navigators, BSO Home & Community Care Clinical Care Coordinators and BSO Client Intervention. The RPN also collaborates with partners in the Community Support (CSS) Sector, the Geriatric Mental Health Outreach Team and other allied health care professionals.
- Position will be employed by Leamington Mennonite Home however physical office is Remote
- Maintain a short-term active clinical caseload of residents with various dementias, delirium, mental health, and other conditions resulting in behavioural changes.
- Review and evaluate the recommendations, treatment, care/service plan in consultation with the resident, care partners, and service providers.
- Develop and maintain an effective working relationship with each LTC.
- Conduct regular speaking engagements and education in-services in member homes and throughout the LTC community in the designated service area.
- Monitors service delivery and when needed, mediates disputes between BSO LTC Lead Team and client LTC homes.
- Ensures regular and consistent communication with BSO program leads system navigator through committee involvement and regular reporting.
- Maintains all required BSO Program documentation as per established BSO pathways and Leamington Mennonite Home requirements.
- Identify the need for additional consultation and/or referrals to other services or specialties.
- Document all findings and interventions in a clear and concise manner in the resident’s clinical record.
- Advocate for and protect the individual rights of residents/clients.
- Maintain current knowledge of psychiatric and geriatric medications within scope of practice.
- Maintain a current knowledge of Alzheimer’s disease and related dementias, mental health, and other conditions. Upgrade skills as required.
- Assist with continuous quality improvement initiatives (i.e., responsive behaviour committees) in LTCHs as appropriate. Participate in care conferences at the request of long-term care homes.
- Perform other duties consistent with the job classification, as required.
- Monday to Friday 8am to 4pm
- Flexibility in work hours required (with the potential for on-call responsibilities and/or weekend work)

**Education**:

- Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (RNAO membership is desirable)

**Experience**:

- experience working with persons affected by Alzheimer’s disease or related dementias.

OR
- experience with residents in long-term care or residents in a long-term care home setting

Other Knowledge, Skills, Abilities or Certifications:

- Demonstrated leadership abilities and experience in leading inter-professional teams.
- Current CPR/Heartsaver Certificate
- Experience or advanced training in the field of gerontology nursing and psychogeriatric nursing desirable/preferred.
- Working knowledge of community resources for geriatric mental health services.
- Excellent organizational skills.
- Demonstrated skills and abilities in developing and delivering education programs.
- Training in Gentle Persuasive Approaches (GPA) and P.I.E.C.E.S.
- Knowledge in PCC/MDS/RAI an asset.
- Demonstrated knowledge and skill working with individuals with various types of dementias, delirium, mental health, and other conditions resulting in behavioural changes.
- Demonstrated effectiveness as a team member through collaboration, respect, and effective communication skills in interacting with clients, family members and other members of inter-professional care teams.
- Ability to conform to shifting priorities, demands and timelines through problem solving capabilities.
- Ability to adapt positively and productively to changes in the work environment.
- Demonstrated client-centered focus and commitment to the principles of the ESC BSO Responsive Behaviours Action Plan
- Proven good attendance record.
- Excellent verbal and written communication skills in English
- Demonstrated advanced computer literacy inclusive of Microsoft WORD, EXCEL, POWERPOINT, OUTLOOK, ZOOM and GOOGLE CHROME.
- Demonstrated effectiveness working with mínimal supervision with proven reliability and trustworthiness.
- Confidentiality and professionalism to be strictly maintained at all times
