**Languages**:English

**Education**: Human services and/or relevant post-secondary education (psychology, social work) in a related field is preferred and/or equivalent work experience/lived experience.

The Intensive Case Manager will provide ongoing support to individuals to exit homelessness as they secure and establish permanent housing. Working within the Intensive Case Management model, they will support their individuals in creating a personalized plan and goals. Intensive Case Managers will meet on a regular basis with the individuals on their caseload and provide the necessary supports for their client to maintain housing stability and meet their personally developed goals.

Community Development
- develop and implement community support and development initiatives to further the YMCA’s mission and vision. Build and foster community partnerships in the delivery of these initiatives which may include leading or participating on committees relevant to this position.

**Security and Safety**:Bondable, Criminal record check, Child abuse registry check, Driver's validity licence check.

**Transportation/Travel Information**:Own vehicle, Willing to travel, Valid driver's licence.
