The Kidney Transplant Team is looking for a Care and Transitions Facilitator (CTF) that thrives within a changing environment and works well with the inter-professional team to advocate for and promote wellness while ensuring the delivery of exceptional client care. The CTF is a practitioner, working within the team that is results-oriented and committed to enhance the quality of patient care within a complex health, social and fiscal environment. As a clinical expert, you will serve as a role model and leader to support care that optimizes patient outcomes and experiences while enhancing the safety, efficiency, effectiveness and accessibility of patient care.

**RESPONSIBILITIES**:

- Care Facilitation:_
- Identifies patient care/safety issues and works to resolve issues and communicate concerns to the unit Clinical Leader/Manager in collaboration with other programs/community teams as required
- Provides input and recommendations to the Clinical Leader/Manager regarding physical space, equipment, supplies, and staffing requirements
- May perform direct nursing care including but not limited to assessment and patient monitoring. Participates in multi-disciplinary rounds and provides input regarding patient and family progress.
- Assist with coordination of Living Donor and Recipients Pairs. Assists with the facilitation of processes from evaluation to surgery.
- Provides oversight and coordination support for patients in work up for deceased donor transplants ensuring that patients are triaged appropriately and tests/appointments are scheduled within the appropriate timeframes targeted by the program.
- Education:_
- Facilitates communication of changes in practice, protocols, legislative standards and guidelines, and other relevant material to Hospital staff/physicians
- Works collaboratively with the clinical team to facilitate evidence-based best practice for patients in the Kidney Transplant program and recommends changes as necessary
- Collaborate in the development and implementation of education programs, patient education materials, website, newsletters and other materials for patients and families and internal and external team members and stakeholders. Initiates the evaluation of programs to ensure they are meeting the needs of stakeholders/patients/families.
- Quality Improvement/Research:_
- Updates knowledge by participating in educational opportunities, reading professional publications, maintaining personal networks, keeping current with relevant standards and best practice guidelines
- Supports the Clinical Leader/Manager and team in the implementation of unit/program based quality improvement initiatives
- Identifies/analyzes care practices & system processes that contribute to/impede outcomes and makes recommendations related to evaluation and redesigning processes
- Promotes the use of research findings into practice through methods such as incorporating information into policies, procedures, reference materials, education materials and teaching sessions.
- Supports research activities, such as the participation, in ongoing qualitative and quantitative research projects that guide clinical practice
- Leadership/Administrative_
- Prepares and maintains quality and operational manuals for the Kidney Transplant Living Donor program, to ensure compliance with Health Canada regulations
- Leads and participates in developing corporate and unit/program policies, guidelines and other practice documents
- Reviews policies, procedures and reference materials and makes recommendations for changes. Updates and communicates changes to staff as approved
- Documents processes, utilization statistics, maintains records /databases and provides written and verbal reports. Provides feedback as required to demonstrate accountability and ensure the quality of clinical education services is continuously improved
- Supports inter-professional collaboration in planning, implementation, evaluation and sustainability of local and corporate initiatives
- Participate in internal and external committees/working groups
- Providing leadership in the absence of the Clinical Leader/Manager.

**QUALIFICATIONS**:

- Regulated Health Professional with a Bachelor of Science Degree in Nursing
- Registration in good standing with College of Nurses of Ontario
- Master's degree in a clinically relevant field (or willing to obtain)
- Minimum 3-5 years of clinical experience with chronic disease required
- CNA certification in Nephrology or equivalent clinical experience required
- Certification as a Diabetes Educator (CDE) considered an asset
- Demonstrated leadership in quality improvement and education, initiatives
- Demonstrated experience as a clinical expert/resource/education for staff
- Demonstrated ability in problem solving
- Excellent communication, facilitation and negotiation skills
- Demonstrated ability to manage competing priorities and balance multiple, disparate tasks
