We have an exciting opportunity for an **Indigenous Health Liaison Social Worker** based out of Fraser Health sites located in** Surrey, B.C,** the central hub for the position.This role gives you the chance to work as part of an Indigenous Health Liaison interprofessional team in supporting Indigenous clients and their families in the Fraser South area.

**Build on your education & career experience as you**:

- Work as part of a multi-disciplinary team as an Indigenous Health liaison for clients, families, service providers, and community agencies to enhance service delivery through consultation and care plans and advocate for client and family expectations.
- Conduct client assessments and screenings that include evaluation of emotional, physical, social, spiritual, economic and mental status of client using information from all available sources.
- Participate in a multi-disciplinary team regarding Indigenous Health; identify clients and refer to appropriate services; communicate with and obtain assistance from appropriate community agencies to ensure the best possible care for clients and families after discharge from hospital.
- Provide education and promote health initiatives/services to individuals, families, and Indigenous communities to increase knowledge regarding self care and whole body health.
- Encourage informed decision-making, personal growth, learning, and empowering the client to improve their quality of life.
- Act in a public relations and advocacy capacity to represent Indigenous Health to medical staff, community agencies, and other service providers.
- Provide consultation to staff for the purpose of enhancing knowledge of Indigenous cultures, contemporary social issues and health beliefs as well as information on social and health services available in the community for Indigenous people.
- Review and adapt relevant information and materials; participate in professional development activities to keep up to date with current trends and research; assist with orientation of new staff, students, and other health care professionals; maintain knowledge of community social work services and resources.
