**About the Opportunity**:
Reporting to the Health Services Manager, Cancer Care, NZ, the Health Care Social Worker provides a diverse range of interventions to clients and their families, enabling them to achieve their optimal level of functioning and health. These interventions include: psychosocial assessments, adjustment therapy, supportive therapy, crisis counselling, resource counselling, and discharge planning and case management.

Health Care Social Workers are actively involved in advocacy, education, program planning, consultation and collaboration on behalf of individuals/groups within the institution and community. The role is multifaceted and dynamic, requiring each Social Worker to work autonomously as well as collaboratively within interdisciplinary teams. The Health Care Social Worker is a licensed professional who practices in accordance with a set of National/Provincial Standards and Code of Ethics and Department Procedures, Policies and Standards.

**About You**:
We would love to hear from you if you have the following:

- Master of Social Work Degree from an accredited university
- Current registration and membership in the Nova Scotia Association of Social Workers and Canadian Association of Social Workers
- Two (2) years clinical experience as a Registered Social Worker required
- Strong counselling and communication skills
- Knowledge of the fundamentals of human behaviour including systems theory, family dynamics, group dynamics and theories of behaviour change
- Experience in oncology, grief and loss, advance care planning, end of life, disability, crisis intervention, caregiving, an indepth understanding of the continuum of care
- Awareness of local community resources and agencies would be an asset
- Leap palliative care training preferred
- Proven self directed leadership
- Advanced leadership training in working with people with developmental disabilities
- Ability to work independently and as part of a team, while effectively planning, organizing, and supervising
- Ability to be flexible and adjust to the changing needs of the service and the prograM/Facility
- Must be able to work alone or with team members in all service settings
- Access to reliable transportation
- Competencies in other languages an asset, French preferred

**Hours**:

- Permanent Full-time Position; 75 hours bi-weekly
- Hybrid position with a combination of work from home and on site

**Compensation and Incentives**:
$37.31 - $49.05 Hourly
