**You are**

A_ Rehabilitation Counselor_. The Rehabilitation Counselor provides individual and family-centered services to adults and/or children and their families. The Rehabilitation Counselor works within a geographic region, out of the designated regional office under the direction of the Program Manager, Rural Services. Service focuses on identifying needs, setting goals, and brokering resources that empower consumers to achieve their goals. While working collaboratively with participants and collaterals, the Rehabilitation Counselor maintains a high standard of professional practice. The Rehabilitation Counselor evaluates the changing needs of consumers and communities to ensure the required services are being offered.

**Your Role**
- Delivery of support and case management services to Manitoba Possible consumers, to facilitate the full participation and equality of persons with disabilities, adhering to Manitoba Possible’s established practice and written protocols. Case management includes:

- intake process
- needs assessment and goal setting
- service plan development, review, and documentation
- referrals, information dissemination and advocacy
- counseling
- Delivery of Provincial Children’s disAbility Service (CdS) and Employability Assistance for People with Disabilities (EAPD) programs to consumers who meet eligibility criteria following program policy and procedure manuals and in keeping with Manitoba Possible values and mission.
- Development of community awareness of disability issues and receptivity to the full participation and equality of persons with disabilities and promotion of the role of Manitoba Possible through networking and educational activities. Examples include:

- Participate on committees
- Organize ASL immersion, disability awareness, or other educational programs
- Consult with organizations regarding inclusion or accommodations
- Contribute observations regarding service gaps and community needs
- Establish and maintain collaborative relationships with the consumer, the consumers’ support network and/or significant others to maximize the consumer’s participation in the assessment of needs and the development of plans.
- Maintain collaborative relationships with community resources, handle inquiries about agency services, ensure that Manitoba Possible services and programs are known, and obtain feedback from the community on Manitoba Possible services.
- Be familiar with and perform as necessary, administrative, statistical, and accounting procedures to ensure the smooth ongoing operation of the office.
- Carry out and maintain all requirements inherent in the Manitoba Possible case management system, CdS policies, EAPD policies and regional procedures.
- Participate in relevant team meetings, special projects, committees, and in-service training related to the responsibilities inherent in the job.
- Participate in the hiring and supervision of staff and students.

**What You Know**
- The Rehabilitation Counsellor will possess a Bachelor of Social Work Degree, or relevant post-secondary education with three years of related experience.
- Experience managing a caseload, and knowledge of services available to children and adults with disabilities.
- Valid driver’s license and access to a vehicle and must be willing to travel within the region.
- Knowledge of ASL an asset.
- Proficiency in French an asset.
