Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario. Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient's independence and dignity. We also provide long-term care home placement services, and facilitate our patients’ access to the community-based services they need.

**Position Summary**:
Under the general direction of the Manager, Home and Community Care Support Services and in accordance with provincial and Champlain Home and Community Care Support Services standards, legislation, policies and guidelines, the Intake/System Navigator for Stroke Rehabilitation is responsible for working with patients and system partners towards integrated and timely access to stroke rehabilitation for stroke survivors in Champlain. This position will facilitate linkages among the continuum of care for subacute stroke rehabilitation services, with the ultimate goal of increasing the capacity of, and access to, services for individuals living with stroke.

**Primary Responsibilities**:

- Work with system partners to ensure timely and appropriate triage of patients post stroke to services that best meet patient needs.
- Perform initial telephone assessments and intake processes for patients referred to the Community Stroke Rehabilitation Program in Champlain.
- Facilitate access to services and supports for stroke rehabilitation as well as other services as per assessed need for individuals with stroke and their families.
- Work with system partners to streamline and standardize referral processes.
- Fosters community engagement through the development of strong relationships with stroke system partners across the continuum of care
- Acts as a resource to other health care providers including hospitals and HCCSS Care Coordinators, as it relates to stroke rehabilitation services that are available across the Champlain region
- Participates in provincial and regional meetings to link with others and to develop possible solutions towards system issues
- On-site work at the Physical Medicine and Rehabilitation Clinic, Bruyère Continuing Care will be required on a regular basis to fulfil job requirements.
- Assist to establish and maintain tracking systems related to stroke rehabilitation co-ordinated access processes
- Performs other related duties, as requested

**Education and Experience**:

- Registered Nurse (BScN or diploma) and registered with the College of Nurses of Ontario; or
- Degree or diploma in Physiotherapy and registered with the College of Physiotherapy of Ontario; or
- Degree or diploma in Occupational Therapy and registered with the College of Occupational Therapy of Ontario; or
- Degree in Social Work and registered with the Ontario College of Social Workers and Social Service Workers; or
- Degree in Speech Therapy and registered with the College of Audiologists and Speech-Language Pathologists of Ontario; or
- Degree in Foods and Nutrition or equivalent and registered with College of Dietitians of Ontario.

AND
- Five (5) years’ experience working with the stroke population
- Includes at least two (2) years working as part of a community based service

**Knowledge**:

- Knowledge of the health care delivery system and community resources within the Champlain region
- Knowledge of Home and Community Care Support Services
- Knowledge of stroke rehabilitation related resources, practice networks and system partners
- Knowledge of medical conditions and stroke related risk factors and stroke related sequelae
- Knowledge of Canadian Stroke Best Practice Guidelines
- Client-centered, multi-disciplinary team approach to client care

**Skills and Abilities**:

- Proficient in the use of computerized processes including the use of mobile (lap top) technology
- Excellent written and verbal communication skills
- Team player with ability to accept and adapt to change
- Ability to maintain a positive attitude in stressful situations
- Ability to organize daily workload in the presence of frequent interruptions
- Proven ability to create partnerships and linkages with system partners
- A valid driver’s license and access to a vehicle is required
- Fluency in English and French is required.
- Vulnerable Sector Check completed within the last six months

**Pay Rate**: Wages are in accordance with the collective agreement between Home and Community Care Support Services Champlain and OPSEU and starting salaries range from $40.409/hr. - $43.169/hr

**Shift**: Monday to Friday, 8:30am to 4:30pm

**Location**: 4200 Labelle St, Ottawa ON

**On-site work at the Physical Medicine and Rehabilitation Clinic, Bruyère Continuing Care will be required on a regular basis to fulfil job requirements.
