**POSITION SCOPE**:
The Community Outreach Worker and Navigator will be compassionate and respectful while providing support to at-risk youth and adults who may be experiencing mental, emotional, spiritual, and physical experiences. The Community Outreach Worker and Navigator will assist members living in and outside the community with navigating government and community services to address the challenges related to homelessness, housing instability, addictions, and mental health. The Community Outreach Worker and Navigator will provide inclusive supports to empower Indigenous women, men, children, youth, families, 2SLGBTQQIA+ by receiving wholistic approached counselling, addiction assessments, mental health and addiction services, after-care services, prevention, and promotion programming.

The Community Outreach Worker and Navigator will patrol local high-risk areas in providing support, information and supplies to individuals in meeting their needs. The Community Outreach Worker and Navigator will assist individuals in finding opportunities, solutions, and resources to address their own social, behavioral, and emotional needs.

**REQUIRED QUALIFICATIONS**:

- Mandatory Requirements_
- Minimum Post-Secondary Diploma in Social Services or Addictions
- Minimum of two-years work related experience within a First Nation Community
- CPIC (Vulnerable Sector) - must be provided annually and at time of offer
- Valid and clean Driver’s License Class “G”
- Must be able to work overtime and flexible for evenings and weekends
- Additional Requirements_

The Community Outreach Worker and Navigator shall be knowledgeable and possess skills or prior experience in the following areas:

- Providing a client centered approach to supporting clients directly, honestly and with empathy
- Demonstrate good listening skills, advocate, and liaise with other service providers in the community and area
- Demonstrate skills and abilities for building and supporting
- Demonstrate an understanding of issues affecting Aboriginal people
- Ability to work independently and as a team player and ensure confidentiality
- Understanding of the barriers and issues facing Indigenous individuals
- Must possess basic computer skills
- Knowledge of community resources and services
- Awareness of issues surrounding addictions, homelessness, Mental Health
- Food safety certificate
- Non-Violent Crises Intervention and/or A.S.I.S.T

**KEY RESPONSIBILITIES**:

- Clinical _
- The Community Outreach Worker and Navigator will help individual(s) or families through the case management model,
- Provide one to one supportive counseling using a wholistic approach with a combination of traditional, cultural, and western supports,
- Assist clients with obtaining their notice of assessments; taxes from all previous years missed,
- Provide Support letters to assist and support individuals who are not connected to O.W or ODSP; O.A.H.S, Homelessness network-CHPI fund,
- Assist with bridging gaps between individuals and their OW and their ODSP workers as well as other community organizations,
- Provide referrals to community resources in the city of greater Sudbury and assist in navigating the services,
- Complete referrals & provide Advocacy Services to improve access to Community Services,
- Network/collaborate with Community Services, Complete clinical administration i.e. EMR-electronic Medical Recording, charting, minutes, brief services, scheduling appointments, etc. and,

- Promotion_
- Prepare and organize materials, brochures, pamphlets.
- Prepare for information sessions, workshops, etc. through shopping, prepare supplies & snacks for individuals & families.
- Provide information regarding Atikameksheng programs, services and opportunities
- Planning and development_
- Ensuring that safety and well-being practices are implemented; this includes being knowledgeable of Universal Care precaution practices, emergency systems and procedures in dealing with crises situations, communicable diseases, duty to report
- Participate in staff development and training
- Coordination and Implementation_
- Developing linkages and maintaining effective working relationships with other community services/agencies to facilitate contacts and services
- Implement Circle of Care (Case Management Model for Atikameksheng Anishnawbek)
- Facilitating connections for youth & adults in promoting awareness and positive connections
- Coordinate, implement and evaluate the Peer Support Program
- Assist
