**JOB PURPOSE**
Trailblazers Life Choices Inc. is a non-profit organization dedicated to providing services to those who are experiencing intellectual challenges, mental illnesses and physical disabilities. You as a part of Trailblazers Life Choices Inc. will work directly with our participants. You will support them to enable independence, self-expression and creativity; help them to access events, activities and maximize participation in the community; increase home living skills and help them to reach their goals and dreams.

**DUTIES AND RESPONSIBILITIES**
What our employees do daily will vary depending on the personal goals and interests of the people they are supporting. Things you will do in your role of a Direct Support Worker include, but not limited to:

- Provide support to a person who is deaf by using American Sign Language (ASL)
- Provide support in all areas of client(s) day program activities
- Systematically analyze activities and job tasks in the community, recreational and work environments so as to plan appropriate training, supervision and supports for clients.
- Provide on-site training which enables the client(s) to learn skills and expectations while integrating naturally into work and community environments.
- Provide advocacy and other support services
- Submit regular updates to Assistant Coordinator/Coordinator regarding the progress and or needs of the client(s).
- Submit incident reports (as they occur) to the Coordinator.
- Serve as a role model for clients.
- Provide flexibility and adaptability to perform other functions as may be required.
- Implement individualized program plans for clients using community resources, activities and work environments
- Identify and develop work placements and recreational activities to ensure maximum integration and skill development
- Conduct job/task analysis to determine basic elements of the job/task to be learned and performed by client(s).
- Break the job/tasks down into teachable parts.
- Introduce job/tasks systematically to clients.
- Identify learning barriers and determine the most suitable training method for the client(s).
- Implement and revise training plans and methods as needed for the participants
- Adapt or modify job duties or work environment to accommodate the needs and abilities of each client.
- Use assistive equipment, devices and fixtures, to enhance clients’ job/activity performance when appropriate.
- Provide personal care and hygiene to client(s) when required.
- Assist client(s) in buying lunch and/or eating when needed.
- Educates natural supports and the communications and support needs of client(s).
- Provide mobility training to assist client(s) in getting to and from work/community sites as required.
- Provide transportation to clients and/or another staff if required by a supervisor (using your own vehicle or a vehicle provided by Trailblazers Life Choices Inc.)
- Provide advocacy and support services to ensure the rights of the client(s) are protected.
- Teach client(s) self-management and independent living skills

**QUALIFICATIONS**
**Mandatory**:

- Completion of an American Sign Language (ASL) certification (level 3) or combination of experience and fluency in ASL
- Be at least 18 years old.
- 1+ years of relevant experience with ASL
- Nonviolent Crisis Intervention Training
- Crisis Prevention Intervention, Medication Administration, Abuse Protocol, Proactive Procedures and Behavioral Strategies, Foundations and Disabilities Training
- Minimum of 3 years of experience providing services to people with disabilities
- Positive attitude and belief that persons with disabilities have the right / responsibility to define and pursue their life choices, and pursue active participation and involvement in the community.
- A valid Class 5 Driver License with clear driving abstract and reliable transportation
- First Aid/CPR Certificate
- Satisfactory criminal record check, driver abstract, child abuse registry check and adult abuse registry check
- Ability to work independently and as part of a team
- Knowledge of basic health and safety rules
- Completion of a high school diploma or equivalency.
- Strong written and verbal communication;
- Good interpersonal skills.

**Preferred**
- Knowledge of regional sign language variations as well as awareness or knowledge of Signed Exact English (SEE)
- Certificate in Disability and Community Support
- Skills in assisting individuals in anger management, daily living, self-help, etc.
- Knowledge of developmental disabilities

**WORKING CONDITIONS**
Direct Support Worker works in a changing environment that adjusts to the needs of the individuals in program services. The environment may be fast-paced with interruptions, distractions and deadlines. Direct Support Worker needs to be flexible and be able to adjust to changing needs or activities.
