**Key Case Workers **are responsible for supporting clients in meeting their housing goals through effective case management including assessments, case planning, coordination & implementation of plan and evaluation of services required to maintain housing. The Key Case Manager is responsible for actively engaging community resources to build a seamless transition for CUPS clients (singles, couples, and families) in accessing the CUPS Housing Program. Key Case Managers navigate through complex, social and government services to aid clients in accessing wrap-around support to achieve housing stability and increased overall wellbeing.
**Organizational Duties and Performance**:
- Following the ‘Housing First’ Model, complete strengths-based assessments with the client and develop short and long term plans that utilize strengths to obtain goals
- Connect clients to services internally and externally needed to achieve goals, following up regularly to assess progress and identify obstacles, accompanying clients in accessing services to ensure needs are met
- Support the client in the navigation of crisis and conflicts with landlords when needed in coordination with Housing Locator
- Remain up to date on changes to programs services and personnel both internally and externally to ensure accurate care plan development, efficient referrals and warm hand-offs of clients to colleagues
- Accurately enter all required data points in organizational & external databases, maintaining up to date client records and participating in program research as required
**CUPS Leadership Activites**:
- Develops and maintains strong community partnerships with community and government services
- Provides respectful, trauma-informed care by assisting the client where they are at, providing information, de-escalation support, and fulfilling crisis support as appropriate and available
- Operate in synergy with CUPS internal departments continuously assessing client needs, gaps in service, care plan modifications and advocating for the voice and self-determination of the client
- Facilitate a healthy and safe work environment through learning and enacting all CUPS health & safety protocols, while maintaining an active awareness of clients and colleagues
- Promote a culture that reflects CUPS vision, mission, philosophies and values
**Stakeholder Relations**:
- Foster excellent relationships with and facilitate case coordination with all client service providers & housing locator to reduce gaps in service
- Develop and maintain collaborative and accountable working relations with internal CUPS colleagues to strengthen communication flow and cross-program understanding
- Provide caring, reliable, trauma-informed care to all clients accessing CUPS to reduce barriers to service and increase community trust
- Share experience and knowledge with students, volunteers and guests interested in CUPS services to further develop CUPS social justice impact & external ambassador network
**Background required**:
- Bachelor’s Degree in Social Services (Social Work/Psychology) with 2-3 year experience or Diploma in Human Services with 3-5 years’ experience within the vulnerable sector
- In-depth knowledge of Calgary social and government services, with previous experience working with Alberta Works, AISH and Calgary Housing
- Excellent conflict resolution & problem-solving skills with demonstrated experience supporting clients with multiple barriers
- Previous experience working in independent/outreach positions preferred
- Demonstrated skill working effectively within a multidisciplinary team
- Excellent case reporting & documentation, with knowledge and understanding of client service outcome tracking and quarterly/annual reporting on case outcomes
- Proficiency in client databases (ex. FCSS, HMIS, etc.) considered an asset
**Position requires**
- Basic First Aid and CPR Certification
- Class 5 Driver’s License with a clear drivers abstract and personal vehicle available to use for work purposes
