**SCHEDULE**: Monday to Friday (8:00am - 4:00pm)

The full time Program Facilitator will provide program Participants with opportunities to participate in meaningful activities while enhancing their skills in areas of; recreational, social-emotional and other life skill areas. Program activities are based on Participants Plan of Care / Individual Support Plan goals. The Program Facilitator will provide support with scheduling delivery of services, administrative support and provide overall support with coordinating activities and communication to Participants and their supports to meet the needs of the Participants and program.

This position also supports the Program Manager with the daily administration of the program, keeping the Program Manager informed of program needs/concerns. The Program Facilitator has the authority to delegate tasks to coworkers, and provide direction to staff in assignments. The Program Facilitator does not have the authority to discipline staff.

PRIMARY DUTIES
- Assist in identifying needs for all individual Participants in the program
- Be familiar with all individualized goals developed from the Plan of Care / Individual Support Plan and assist as necessary
- Maintain monthly schedules for in-person, in community and virtual activities for all individual Participants
- Ensure all individuals are safely transported to community outing, bus/train and family homes
- Ensure a high level of quality care is maintained at all times
- Use the least intrusive procedures necessary to maintain a stable environment
- Maintain a respectful attitude towards individuals at all times
- Prepare & implement life skills training programs for each individual under their care, in order to maintain or increase independence as identified in the Plan of Care/Individual Support Plan
- Provide support to Participants as needed, focusing on the individual's self-identification of issues and problem solving
- Ensure all required paperwork is completed thoroughly and on time
- Act as liaison with family, school/work, and other professionals
- Coordinate and attend all applicable meetings for program individuals, ensuring that all updates are completed by staff and documented accordingly (monthly, bimonthly, annually etc. depending on the Plan of Care/ Individual Support Plan). Follow-up on all identified goals
- Monitor petty cash expense/budgets & complete reports as required
- Keep self and other staff up-to-date on individual protocols & goals
- Ensure all individual files are up-to-date and organized (annual sign offs, consents, safety plans, bathing protocols, behavior plans etc.)

QUALIFICATIONS
- Post-Secondary Degree/Diploma in Social Services or related field
- Experience with adults who are Deaf /Hard of Hearing/ Deaf-Blind and have a developmental disability and/or other possible disabilities (ex: Mental health, physical disability)
- At least one year experience in related field
- Understanding of Deaf Culture and experience with the Deaf community
- American Sign language skills to communicate with program individuals in their preferred language and with colleagues who also communicate in ASL
- Proficiency written English communication
- Experience in working in an environment that uses Safe Management practices
- Proven ability to work well in a team, as well as independently
- Has experience in computer literacy (e.g. Words, Excel, Outlook)
- French language/LSQ an asset
