**Social Worker (MSW)**

- Provide direct counseling, therapeutic interventions and education and to individuals, couples, families and children
- Conduct comprehensive Social Work assessments
- Design individualized treatment plans based on presenting concerns
- Develop and facilitate groups
- Function as an effective member of a multidisciplinary health care team
- Liaise and consult with relevant community agencies and service providers
- **Qualifications**_
- MSW
- Specialization with counselling of children preferred
- Specialization clinical
- Current Registration in good standing with OCSWSSW
- **Experience**_
- Experience in a primary care mental health setting preferred
- Training and familiarity with evidence-based practice models/theories such as: cognitive behavioural therapy, brief solution-focused therapy, motivational interviewing, crisis prevention and intervention, addictions counselling theories and others
- **Familiarity and comfort dealing with**:_
- Anxiety, depression, stress management, divorce /separation, injury/recovery, job loss
- Life cycle issues (with a focus on work with adults)
- Couples families, and children
- Group facilitation
- Health promotion activities
- Works independently within a multidisciplinary health team
- Has positive and collaborative interactions with social workers, physicians, nurses and other staff
- Has excellent computer skills and experience working with an Electronic Medical Record
- Has excellent oral and written communication skills
- Has the skills necessary to expand and strengthen community partnerships
