
Bilingual Rehabilitation Referral Consultant

**Job Description**:
We are looking for a team of **_Rehabilitation Referral Consultants - Centralized Referrals._**
We’re looking for a team of five rehabilitation professionals to work as key members within our newly created Centralized Referral Team.
In this role, you’ll be responsible for reviewing the claims sent to Disability Intervention Services (DIS) by Disability Case Managers for the appropriateness of rehabilitation services. You will assess the rehabilitative needs of plan members who are disabled and determine if through rehabilitation interventions the plan member would be capable of returning to work. Appropriate cases will be assigned to Rehabilitation Consultants and if not appropriate for rehabilitation services, you will provide guidance to the Case Manager.
At times, the Referral Consultant may also maintain a caseload of active files. You will manage complex disability cases in a pro-active manner with the goal of achieving a level of functional improvement that supports the earliest possible return to employability. You will work directly with the plan member as well as their employer, physician and treatment providers to achieve the required level of functional improvement to support the employability goal.
As part of managing your caseload, you will develop, implement, and monitor the progress of approved rehabilitation service delivery plans based on claimant needs and contractual provisions. Research of appropriate treatment resources as well as active management of service providers. A key focus on employability goals is a pivotal responsibility in this aspect of the role.
A third aspect of the role will be to engage in special projects to review files and identify DIS intervention opportunities with the goal of improving overall claims outcomes.
This role is mainly a work from home position.

- Assess the potential for incoming referrals for rehabilitation services considering the potential to impact on the duration of disability of complex claims. Making the decision to accept or decline the referral.
- Ensure timely assignment of work by reviewing and assigning work across the national team, giving consideration into existing caseload as well as customer and regional needs
- Provide guidance and feedback to Case Managers when declining a referral
- Assess the potential for rehabilitation services to impact on the duration of disability of complex claims through file reviews as well as telephone and/or face to face interview of claimants, employers and involved treatment providers
- Develop realistic, cost-effective rehabilitation/ return to work plans for actions focusing on return to work
- Oversee counseling and/or referral services as necessary regarding identified needs of plan members. (disability adjustment factors; treatment; vocational solutions; community resources etc.)
- Maintain follow up contact with the plan member, focusing on barrier resolution and functional improvements made, while providing claimant support during the intervention period
- Maintain proactive follow up with all stakeholders, including policyholders and treatment providers with the focus on returning to employability
- Prepare written reports detailing findings and recommendations
- Track rehabilitation activities and document case files electronically
- Participate in projects to enhance the service, quality, and outcomes of DIS interventions. This work includes file reviews, analysis and reporting
**What you will bring**
- A university degree in a discipline related to rehabilitation with experience in health or social sciences preferred
- 5 years of experience in insurance rehabilitation or the field of rehabilitation preferred
- Excellent communication and interpersonal skills (e.g.relationship building, adaptability, decision making, negotiation, influence, conflict management)
- Excellent organization skills, with experience working in a fast paced environment, requiring regular re-prioritization and critical analysis/ assessment skills
- Mental Health experience an asset
- Fluency in French and English both oral and written are an asset
- Ability to accommodate a home-based office
- Valid driver’s license and vehicle are necessary due to travel requirements
