***:Working under the general supervision of the Director of Programs, the Community Wellness Worker is responsible for servicing the broad needs of an assigned caseload of individuals and families through a process of direct contact, situational evaluations and needs assessments, formulation of remedial strategies to meet the overall constituent needs in an effective and respectful manner, and the objective monitoring of the overall effectiveness of these activities. Additionally, the Community Wellness Worker will initiate and participate in community outreach and awareness programs designed to increase awareness of the Abinotci Mino-Ayawin Program within the Aboriginal Community. It is expected that the CWW have the ability and skills to work safely with constituents who may be hostile, oppositional and volatile, while at all times maintaining composure and behave in a professional and dignified manner. The Community Wellness Worker shall ensure that all activities are undertaken in a manner that is consistent with, and complementary to, the Vision Statement, the Principles and Values and the policies and procedures of the Aboriginal Health & Wellness Centre.

DUTIES & RESPONSIBILTIES:

- Utilizing the principles and values of the Medicine Wheel as a guide, and supplemented by information obtained through the intake process, develop, implement and conduct home visitation interactions with assigned constituent(s) for the purpose of objectively observing and validating the current situation, assess and identify both immediate and long-term constituent needs, and formulate acceptable action plan(s) to address these concerns.
- Maintain ongoing contact and interaction with assigned constituent(s) on a regular basis for the purpose of initiating necessary action or activities, monitoring progress of the action plan(s), and provide necessary interim support, while constantly encouraging personal empowerment, independence and self-reliance.
- Ensure that all assigned constituent(s), as well as members of the urban Aboriginal community as a whole, are aware of, and have ready access to, the services of the Aboriginal Health & Wellness Centre, community based groups and agencies providing child and family support, and other Aboriginal agencies in order to meet the needs of constituents in a structured, comprehensive manner.
- Maintain a thorough and accurate record of all constituent activities and interactions, case notes, resource reviews and service research, and relevant activity statistics for peer review and historic purposes.
- Interact with other community resources in order to promote awareness of the services offered by both the Aboriginal Health & Wellness Centre and other agencies, the means by which those services can be effectively accessed, and methods of effectively integrating those services / resources with those currently provided under the Abinotci Mino-Ayawin Program.
- Plan, facilitate and assess group activities and workshops that provide direct contact, education, enlightenment and support to constituents of the Abinotci Mino-Ayawin Program and other community resources.
- Other related duties as assigned

**QUALIFICATIONS**:

- University graduate in Social Work, Social Sciences, or a combination of education, training and related practical experience deemed equivalent by the Centre.
- Excellent communication and interpersonal skills including a demonstrated proficiency in conducting individual / group counseling, leading group presentations and group facilitations and effectively dealing with situations of conflict and stress.
- Demonstrated experience and ability to perform independent fieldwork with primary emphasis on the development and maintenance of effective direct constituent relations and maintaining accurate case records.
- Possess a current Province of Manitoba Driver’s License and a suitable vehicle for work-related transportation.
- Demonstrated ability to perform multiple tasks concurrently within a fast-paced, high stress environment.
- Ability to communicate in an Aboriginal language(s) and/or previous exposure to Aboriginal communities and cultures would be a definite asset.
- Demonstrated capacity to function effectively in a working team environment.
