**Substance Use Health Coordinator**

**SUPERVISION**:
Under the direction of the Executive Director The Substance Use Health Coordinator is responsible for the effective and efficient delivery and administration of the Substance Use Health Program in accordance with all relevant policies and procedures.

**JOB SUMMARY**:
The Substance Use Health Coordinator’s primary responsibility is to ensure the development, implementation and stability of the program by assisting individuals in addressing substance and minimizing the negative effects for the individual, and community. Confidential services, education, support and promotion of healthy lifestyles and choices are part of the service deliverables.

**QUALIFICATIONS**:
1. Preferred degree in Addictions Counselling, Social Work, or in a related human services field, with specialized training or experience in the area of substance use issues.

2. Related work experience in the area of substance use and completion of assessments.

3. Strong knowledge and awareness of trauma informed approach to substance use.

4. Experience working with high-risk clientele with substance use issues in a helping capacity.

5. Experience planning and leading educational, one-on-one and/ or support groups. Ability to plan, coordinate and deliver educational and prevention workshops.

6. Self-motivated individual with the ability to work with mínimal supervision in a team-oriented setting.

7. Excellent case and file management skills, stress management and time management skills, ability to meet deadlines.

8. Ability to engage people to build trust and rapport; effective verbal and listening communication skills.

9. Strong written and oral communication skills.

10. Group facilitation, team building and decision-making skills; ability to problem solve and take initiative.

11. Effective crisis intervention skills; ability to provide individual counselling.

13. Ability to work flexible hours, including evenings and/or weekends.

14. A clear criminal reference check (CPIC) and Vulnerable Sector Clearance is a condition of employment.

15. Must have a valid Ontario Driver’s License and reliable transportation.

16. Able to incorporate an intersectional approach to gender, race and discrimination, and aware of subsequent intergenerational impacts affecting Indigenous women and gender diverse individuals.

17. Ability to supervise and direct a PWLE team.

**JOB RESPONSIBILITIES**:
The Substance Use Health Coordinator shall have responsibility for the following:
1. **Case and File Management**:

- Maintain up-to-date client files that track and evaluate participation and progress in the program.
- Complete intake process with clients.
- Complete client substance use assessments utilizing appropriate assessment tools (GAINS).
- Schedule appointments and make referrals.
- Be knowledgeable of and work with community agencies and referral sources who will be in a position to assist clients in meeting immediate and long term goals.
- Provide one-on-one supports, including counselling, peer and/or group support.
- Initiate and participate in case conferences as necessary to ensure client needs are met.
- Provide referrals and advocacy services to individuals experiencing substance use issues such as relapse prevention, depression, anxiety, concurrent disorders, etc.
- Advocate on behalf of clients and provide ongoing liaison services and follow-up to ensure individual needs are being met.
- Schedule visits with clients and follow through with case management.
- Develop annual service plans.
- Work with EFSNWO programs to promote a coordinated client service approach to ensure client needs are addressed.
- Assist clients in preparing treatment and aftercare plans.
- Arrange for emergency assistance as required in areas pertaining to crisis intervention.
- Model behavior in a manner that serves as an example of positive cultural values, attitudes, beliefs, and actions.
- Provide and/or arrange for translation services for individuals and families.
- Assist in arranging transportation for clients as needed.
- Assist client in aftercare treatment program planning, and in preparing treatment and travel plans.
- Provide participant-based activities that include public education / awareness campaigns, workshops, presentations, events that address substance use issues.
- Support the development of healing circles, peer support groups and self-help processes to assist in community healing.
