The Intervention Worker is responsible for the integration, support, and referral of individuals or families soliciting services from CDNBCA. The Intervention Worker develops the action steps for client’s community public support plan (CPSP) to access public resources to address various socio-economic challenges. He/she works within the guidelines, policies and mission of the organization and will be accountable and responsible for specific projects as assigned.
**MAIN TASKS**
- Support clients to complete the action items in their intervention plans developed with the Community Access Support Service (CASS);
- Accompany high risk families and/or individual to access internal/external services within the timeline outlined in their community public support plan;
- Act as liaison and advocate for client (families) with public agency workers (e.g. social workers, school administrators, medical professionals, etc. );
- Collaborate with Strengthening Families Program Montreal (SFP MTL) staff to provide crisis intervention service to families referred by youth protection agency.
**RESPONSIBILITIES**
- Maintain up to date files of clients and/or families in electronic system;
- Make weekly calls to clients and/or families to confirm attendance to appointments or programs, determine reasons for absence (if any), and check general welfare;
- Provide follow-up and complete updates in the system on progress of the action steps assigned to each client;
- Collaborate with SFP MTL team to plan and execute strategies to recruit families for SFP sites and CASS services.
- Develop working relationships and liaise with public agency workers (e.g. CLSC, CIUSSS, Batshaw Youth Services) workers and professionals with the social service sector;
- Be familiar with and refer families to community resources in proximity to their residence;
- Attend and participate in seminars, meetings, and consultations as required.
**KNOWLEDGE, SKILLS AND ABILITIES**
- Degree in Social Work or related field i.e. human services, health or education or a minimum of 5 years relevant work experience
- At least 2 years’ experience in non-profit services
- Fluently bilingual with excellent communication skills both verbal and written
- Excellent computer skills and proficient in excel, word, outlook, explorer, access and cloud based systems
- Strong problem solving and leadership skills
- Ability to interact with people of all ages and cultural backgrounds
- Ability to work independently and as part of a team
- Ability to work flexible hours
- A demonstrated commitment to high professional and ethical standards
- Excels at operating in a fast pace, community environment
