- **The BSO Social Worker will oversee the coaching, mentoring and modeling strategies that are to be implemented to manage responsive behaviors**_. By ensuring a commitment to client centered resident care, the lead will foster a culture that is positive, creative, respectful, flexible, and enthusiastic.

**Duties and Responsibilities**
- Works collaboratively with care partners and consultants serving Long Term Care such as HNHB Placement Services, Alzheimer Society PRCs, Specialized Geriatric Services, Community BSO services, LTCH BSO Mobile, BSO Transitional Leads and discharge planners and other health service provided as required
- Completes assessments based on best practice, and assists the resident in the development of goals and care planning strategies
- Works with the specific LTCH leadership team to support the needs of the individual and with consent from the resident reports on the care plan, assessment and findings to the same
- Participates in Continuous Quality Improvement practices as identified by the BSO Program
- Supports the resident by providing task specific support and counselling
- Provide recommendations on bio-psychosocial and non-pharmacological interventions to support resident goals in collaboration with the resident/ their family / significant other and the interdisciplinary care team
- Schedules visits with the resident and care team within assigned hours of the BSO Mobile program and locations as agreed upon with the Sub Region manager in a case-based manner
- Ensures care strategies are evaluated and adapted in consultation with the resident and those within the resident’s circle of care
- Provides guidance and support that is person-centered and respectful of the intrinsic worth of individuals
- Identifies themes and opportunities for educational in-services in collaboration with the LTCHs, other service providers, outreach services/teams and experts in the field.
- Communicates professionally with clients, families, co-workers and others for the purpose of meeting client needs
- Collaborates with LTCH staff/leadership, families, and service providers in a professional, courteous manner and keeps the BSO Sub Region Manager, Behavioural Lead / Clinical Coach informed of any changes or concerns as needed
- Participates in all BSO sub-regional staff meetings, education and/or training as required by the host employer St. Joseph’s Villa
- Supports successful resident care transitions by facilitating care plan recommendations identified by the BSO Transitional Lead, hospital Clinical Leader, community partners and BSO family of services in shared clients/ residents
- Documents visit notes and assessments according to BSO standards, within the resident’s chart and according to College of Social Work Standards
- Supports capacity building of staff and staff mentorship through modeling and coaching of strategies/ approaches to staff as appropriate
- Carry out all duties with appropriate attention for resident/client safety
- Complete other duties as assigned by the BSO Mobile RN Manager
- In line with the OCSWSSW, the Mobile Social Worker will provide Social Work services in a manner that is professional and promotes excellence

**FORMAL EDUCATION**
- Degree in Social Work (Registered, BA or Masters) in good standing and must maintain College Registration at all times

**OTHER KNOWLEDGE, SKILL AND EXPERIENCE**
- At least 2 years’ experience; Long Term Care, specifically working within a Mental Health setting or setting providing services to older adults with responsive behaviours related to dementia, mental health or other factors
- Training in Montessori, GPA, adult mental health is an asset
- Demonstrated leadership and ability in coaching, teaching, mentoring staff
- Ability to adapt positively and respectfully to changes in work environment and work location
- Valid Drivers’ License
