**Job Summary**
The Licensed Practical Nurse (LPN) assesses, plans, implements, and evaluates nursing care for their work assignment in accordance with competency guidelines within the Professional Standards outlined by BCCNM.
**Responsibilities and Duties**
- Assesses and monitors residents/patient’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs.
- Where appropriate encourages residents/patients’ participation in decision making in regards to their plan of care and activities of daily while maintaining independence as much as possible.
- Where appropriate encourages residents/patients’ participation in providing their own personal care. Encouraging them to be as independent as possible to towards achieving an improvement in their care and assistance needs.
- Contributes to the development and modification of the individualized plan of care, by providing input into interdisciplinary care conferences, medication reviews and other meetings as required.
- Under the guidance of the DOC or Direct Nursing Supervisor and in accordance with the established policies and procedures, administers prescribed medications and treatments.
- Performs nursing care and procedures and evaluates outcomes; records observations and reports problems and/or changes to the Nurse Case Manager; Pro Vita Manager, Registered Nurse or Direct Nursing Supervisor.
- When required performs admissions, discharges, and transfers of residents/patients according to established Pro Vita policies and procedures.
- Observes, monitors, documents and updates information about resident’s physical and psychological condition, care, and nursing procedures.
- Supervises and provides direction to the nursing care team including reassignment of workload.
- Provides and assists with personal care to residents/patients’ as required.
- Assists residents/patients’ with meals, ensuring they receive adequate nutrition according to individual care plans and reporting changes in appetite/food intake to Registered Nurse and/or Dietician.
- Completes Serious Incident Reports as required.
- Participates in staff education and provides orientation of students and new personnel to job requirements, equipment and routines.
- Assumes responsibility for own continuing education and professional development; maintains current knowledge of trends and concepts in gerontological nursing.
- Promotes and maintains positive communication with all residents/patients’, staff and the public.
- All safety rules and safe work procedures are adhered to at all times.
- Reports all hazards, incidents, near misses and workplace injuries to the Nurse Case Manager or Nursing Supervisor immediately and in writing.
- Does not participate in unsafe situations (horseplay), and corrects any unsafe conditions if able.
- Actively participates in all Health and Safety initiatives and make suggestions to improve Workplace Health and Safety.
- Performs other related duties as assigned.
**Qualifications**
- Valid LPN license with BCCNM
- Maintains Valid First Aid and CPR C
- Experience with Point Click Care is preferred
- Meets the physical requirements for the role.
- 1-2 years’ experience in a long term care facility is preferred.
