**Summary Role**:
**Duties and Responsibilities**:

- Elaboration of intervention plans (I.P.) and of individualized service plans (I.S.P.)
- Participate in the awareness initiatives and health education of the community.
- Develop and maintain a client caseload.
- Ensure that the caseload is well and ethically managed
- Maintain updated case notes and statistics, as well as client data-base in accordance with departmental policies and procedures.
- Carry out clinical interventions and follow ups that improve adults and vulnerable families’ wellbeing.
- Promote optimal healthy, holistic lifestyles and enhance relationships and improve individualized support system through regular visits.
- Receive, evaluate and assess the needs and of the clientele. Incorporate traditional healing when beneficial to the client’s progress
- Promote Timiskaming First Nation Health and Social Services as well as other TFN Programs and Services.
- Promote an atmosphere in which adults and families are encouraged to take responsibility for meeting their personal needs.
- Attend seminars, workshops, conferenced, meetings related to First Nation health and wellness issues.
- Intervene and refer in crisis situations.
- Develop, promote activities and maintain a contact network between First Nations organizations and partners (internal and external).
- Collaborate with agencies, establishments and families serving similar clientele.
- Develop links with and utilize the resources in the community.
- Attend Court as a witness as required, in court proceedings, involving individuals and families.
- Facilitate and participate in various committees when required.
- In collaboration with the program’s frontline workers, analyze individual services (needs and priorities) provided to community members that could be converted into collective and community services and support their implementation through appropriate interventions.
- Manage the human, financial and material resources.
- Perform other related duties as requested by his immediate supervisor.

**Requirements / Qualifications**:

- Post-Secondary Diploma/Degree in Social Services and one (1) year plus experience
- Valid Drivers’ License
- Knowledge of Timiskaming First Nation (TFN) services and programs,
- Knowledge of relevant challenges and issues affecting First Nations
- Vulnerable sector check must be attached to be considered (or must be able to attain as soon as possible)
- Must have received, or willing to receive the Covid-19 vaccination by date of hire to be considered. Proof of vaccination required.
- Knowledge of traditional medicine and a holistic approach to community health and wellness
- Knowledge of health legislation, theories, practices and procedures and medical terminology
- Ability to work effectively in a variety of settings and in a cross-cultural situation
- Organizational skills with the ability to multi-task and prioritize
- Ability to work independently and ability to work in a team oriented, multi-disciplinary environment
- Must be knowledgeable of the First Nation culture.
- Ability to represent TFN with a professional image and maintain confidentiality
- Ability to communicate in the French and Algonquin languages is an asset
- The ability to provide services according to a work schedule determined by the employer and according to the needs of the individuals served.
- Strong crisis and suicide intervention skills;

**Job Types**: Full-time, Fixed term contract
Contract length: 12 months

**Salary**: $24.78-$35.67 per hour

Schedule:

- Monday to Friday
