A Project Management role with this team offers an opportunity to work with partners from across the Mississauga health system to engage our communities and members, redesign care with the person at the centre, improve how people navigate across our system, and strengthen the foundation of the Ontario Health Team to make an impact for the people in our communities.

As part of being recognized by the province as one of the initial 12 Ontario Health Teams (OHT), Mississauga Health is working towards improving person care and outcomes in priority areas, including integrated clinical pathways, and prevention and early detection of chronic illnesses. In collaboration with our partners, we are co-designing and implementing integrated clinical pathways for chronic diseases, beginning with Heart Failure (HF) and Chronic Obstructive Pulmonary Disease (COPD). These integrated pathways aim to coordinate and deliver person-centred services for people experiencing chronic diseases and connect care seamlessly along the person’s entire journey. Engaging the primary and community care network will be foundational to improving patient outcomes through this work, advancing provincial clinical priorities and demonstrating improvements to patient experience and outcomes.
**Here’s what you’ll get to do**:
- ** Facilitate and enable integrated project delivery** by taking a systems-approach to planning, decision-making, execution, risk management and overall reporting for an integrated care model for chronic diseases, beginning with the implementation of integrated clinical pathways for Chronic Obstructive Pulmonary Disorder (COPD) and Heart Failure (HF)
- **Oversee all elements of the project approach, workplan development and execution** (develop, track and execute actions per project timelines) as well as implement necessary processes and practices to ensure timely delivery on project milestones and successfully achieving the project's goals;
- ** Support team leadership with strategic oversight** and guidance to achieve project deliverables, ensuring alignment to the overarching strategy and integrated goals of the OHT, and identifying and seeking endorsement of key deliverables and decisions and mitigation of risk;
- ** Engage and motivate a broad set of stakeholders** around a shared purpose and collaborate across partner organizations to develop and deliver on initiative workplans to collectively achieve milestones.
- **Responsible for key relationship management** across various stakeholders including people with lived experience, caregivers and community members, as well as broad stakeholders spanning health care providers, teams and leadership from partner organizations across the health and wellness system (primary, community, specialty, and acute care, regional, municipal, and social services)
- Support broad multi-sector project teams through all phases of project life cycle, providing coaching, motivation and guidance throughout the duration of projects;
- Engage and communicate with stakeholders on an ongoing basis and ensure changing resource and project requirements are understood and met, and facilitate change management through engagement of our communities and members
- Liaise with primary care, community health partners, specialty and acute care including regional, provincial and/or private sector partners to ensure the successful completion of project deliverables
