**PURPOSE**

The Transitional Housing Program Case Worker provides a safe and secure environment for residents.

Provides compassion, encouragement, life skills coaching, and practical support and conducts programs that will assist residents to successfully re-integrate into the community.

**WORKING RELATIONSHIP(S)**

Reports to the Transitional Housing Program Coordinator
- Participates as a team member of The Salvation Army Harbour Light Ministries
- Works closely with community partners

**RESPONSIBILITIES AND ACTIVITIES**
- Monitors the activities of the residents and maintains availability for ongoing case management
- Manages a resident case load up to a maximum of 40 residents when program is operating at full capacity
- Accurately records day-to-day happenings using appropriate methods; maintains case notes and communicates appropriate information with other staff in Transitional Housing as well as within the Toronto Harbour Light Ministries, as needed
- Provides life skills coaching and support for clients
- Facilitate program groups and meetings as necessary
- Provides referrals and acts as a positive role model for the residents
- Liaises with referral sources and other external community agencies as needed (e.g. Ontario Works, shelters, Ontario housing, police in urgent situations)
- Request and test client urine samples if needed
- Conducts unit inspections as needed
- Handles the admission, orientation, evaluation of residents' understanding of rules, and discharge procedures; ensures the ongoing enforcement of policies, ensures residents participation in the program
- Assist residents to find permanent housing
- Serves as a representative of The Salvation Army Harbour Light Ministries maintaining and developing relationships with community partners.
- Performs other related duties as assigned

- Be in agreement with and willing to support our Mission Statement and treatment philosophy
- Maintain a drug, non-smoking and alcohol free lifestyle while on Salvation Army property and on duty
- Model appropriate dress that reflects a professional environment
- Participate in all staff meetings by attendance and participation
- Participate in regular direct supervision with immediate supervisor and implement improvement in performance as directed
- Participate in yearly formal evaluation
- Represent The Salvation Army Toronto Harbour Light Ministries in a professional manner at all times
- Be punctual and work all hours scheduled, advising immediate supervisor when this is not possible

**The above responsibilities are to be discharged in accordance with The Salvation Army’s Mission Statement, in a professional manner.**

**QUALIFICATIONS/ SKILLS/ ABILITIES**:

- Undergraduate degree or diploma from a recognized institution (in one of the human services professions), Professional Designation an asset
- Minimum 3 years’ experience working in mental health/social services field
- 1-2 years case management experience, housing experience an asset
- Excellent time management/organizational skills
- Knowledge and experience in conducting assessments, individual case management, and treatment modalities
- Knowledge of culturally competent practices
- Ability to maintain client records / files according to agency standards
- Ability to work individually and as part of a team
- Ability to work collaboratively with community partners
- Knowledge and experience with data management systems
- Strong work ethic, initiative and commitment to delivering positive outcomes through reduction in housing loss
- Successful police check as per Salvation Army Policies and Procedures
- Training and experience in counselling in the following areas considered an asset:
Ø Trauma-informed practice

Ø Mental health recovery skills

Ø Crisis/conflict management

Ø Assessment and referral

Ø Harm Reduction strategies and resources, Relapse prevention and Concurrent Disorders

- 37.5 hours per week - Monday through Friday afternoons and evenings
