The Lead, System Strategy, Planning, Design & Implementation team of Ontario Health North West Region is responsible for designing a Regional strategy for delivering on Ontario Health and Ministry priorities in the Regional context. The team leads the coordination, design, and implementation of strategic initiatives, including collaboration with the Ontario Health Team (OHT) program, chronic disease prevention and management, community paramedicine and rehabilitation programs, and strategic partnership initiatives.
The primary role of the Lead is to plan and deliver strategic projects and integration activities on behalf of the North West Region with a focus on regional chronic disease prevention and management program, such as diabetes, stroke, cardiovascular, renal, cancer service and programs related to rehabilitation and community paramedicine. This includes engaging many diverse stakeholders (i.e., patients and caregivers, health service providers, internal teams across OH, the ministry) to support the development, implementation and evaluation of strategic and operational priorities. The Lead will need to understand and interpret data to design targeted interventions that support system change. The Lead will also support evaluation activities by helping to develop key performance indicators and reporting requirements to assess impact.
Here is what you will be doing:
Health system planning and integration- Works across sectors and outside of healthcare to identify opportunities for system transformation and integration, particularly related to planning for chronic disease programs, community paramedicine and rehabilitation services.-
- Acts as key contact for regional chronic disease programs and community paramedicine and rehabilitation programs, and leads discussions/activities to support program development- Utilizes data and evidence to help identify service gaps, risks, and challenges and engages appropriate partners to develop and implement solutions- Facilitates engagement with key stakeholders to advance work plans that will achieve measurable progress in priority indicators and align with regional and provincial priorities- Supports evaluation activities by helping to develop key performance indicators and reporting requirements in order to measure and assess impact.
Develop and lead stakeholder relationships- Develop and maintain internal OH relationships and North Region team relationships (e.g., Digital, Analytics, Clinical, Performance/Finance) to advance regional strategy and integration priorities and ensure regional alignment- Provides advice and support, as required, to North Region planning staff, health service providers, and other partners on initiatives and opportunities that will advance strategic health system goals- Leads discussions in department and stakeholder meetings, working groups, committees, advisory groups, council and/or other relevant meetings which may have an impact on project /corporate initiatives- Acts as the key North Region representative at various project committees to advance team goals- Initiates and develops effective working relationships with external stakeholders (e.g., Ontario Health Teams, HSPs, broader health system partners and patient, family, and caregiver advisors) to keep up to date on current/emerging initiatives and issues
Project management- Develops, manages and executes on work plans, including effectively tracking milestones, project risks, and project status- Implements a data driven approach to drive strategic opportunities- Works collaboratively with project support resources from both within and outside of the organization to align planning processes- Utilizes strong knowledge of project implementation to lead large scale projects and manage communications with relevant stakeholders
