WellFort is seeking a **Social Worker / Clinical Counsellor** to join our team
**1.0 FTE contract position until June 30, 2024 (0.6 FTE Bloom and 0.4 FTE Primary Care)**
- **This full-time contract position will support the Bloom Clinic (3 days/week) and Primary Care (2 days/week)**_

WellFort Community Health Services is a non-profit, values-based organization based in Peel. We provide a wide variety of comprehensive care including primary care, dental, mental health, and chronic disease management with a focus on illness prevention, health promotion and community development. We work with residents and other agencies to increase community capacity.

Bloom Clinic is a program of WellFort Community Health Services and is committed to providing treatment, care, and support to individuals living with, affected by, or at risk of Hepatitis C (HCV) and/or HIV across the Region of Peel. Bloom Clinic is committed to providing community-based testing, treatment, and support to people who are experiencing homelessness, people who use drugs, people who are incarcerated or otherwise involved in the criminal justice system, and Indigenous community members.

The Social Worker / Clinical Counsellor works as a part of an interdisciplinary team functioning both independently and collaboratively with other staff in clinical, counselling and health promotion activities supporting people living with or at risk of acquiring HIV and/or Hepatitis C(HCV).

The Social Worker / Clinical Counsellor will lead in providing therapeutic treatment services and development, monitoring, and evaluation of programs and services for WellFort priority population groups including: youth; families who are under-resourced; people who are homeless, under-housed, or at risk of homelessness; people who are dealing with substance use and/or mental health issues; isolated seniors; newcomers and members of racialized communities.

**What you will do**:

- **Bloom Clinic**_
- Provides ongoing counselling and health information to individuals living with HCV and/or HIV as needed to assist in coping with various issues including, new diagnosis, social and emotional difficulties, treatment preparation, medication adherence, substance use, etc.
- Provides crisis intervention, as required.
- Accompanies clients to appointments, when appropriate, to act as a support and advocate.
- Acts as a resource and advocate for clients struggling to navigate various health and social service systems (e.g. OW/ODSP, housing supports, legal aid, etc.).
- Ensures appropriate treatment is delivered by maintaining client notes and correspondence within organizational standards and the safeguard of confidentiality.
- Plans, implements and evaluates the day-to-day functions of Bloom’s health promotion, awareness and prevention initiatives.
- Engages in regular HIV and HCV outreach testing activities.
- Acts as a resource to staff and community organizations on HCV and HIV education, awareness, and prevention.
- Creates and facilitates education sessions focused on needs of priority population (e.g. harm reduction, naloxone, transmission risk, social isolation, mental health, housing, etc.)
- **Primary Care**_
- The Clinical Counsellor carries a caseload of individuals, couples and families for brief therapy (8 - 12 weeks)
- Provides therapeutic counselling services to clients experiencing problems of an individual, couple or family, environmental or social nature, from assessment to termination, and crisis intervention as needed
- Uses various techniques in therapy depending on the needs of the client such as CBT, DBT, Solution Focussed, Behavioural and Humanistic
- Works from a strengths-based model that recognizes and values the skills, strengths and uniqueness of every patient
- Ensures appropriate treatment is delivered by maintaining client notes and correspondence within the safeguard of confidentiality and PHI required by health care agencies
- Ensures holistic comprehensive treatment for clients by being an active participant of the interdisciplinary team within our shared care model
- Offers support, psychotherapy, chronic disease self-management and psycho-educational groups using adult teaching and client driven techniques based on the needs of the group
- Develops group cohesion, goals, and appropriate interventions and arrange for follow-up as needed
- Arranges and follows up with case consultation and case planning with other members of the interdisciplinary health care team at WellFort
- Develops collateral contacts with other agencies and institutions in which the client is involved to facilitate comprehensive case planning
- Ongoing evaluation and modification of the plan of care
- Provides services to clients at outreach locations
