Sous la responsabilité de la direction générale, vous serez au cœur de notre projet de gériatrie sociale dans le quartier Mercier-Est. Vous jouerez un rôle clé en encadrant une équipe dédiée et en tissant des liens essentiels avec la communauté et le réseau de la santé pour garantir une meilleure continuité des soins et services pour les aînés.

**Responsabilités principales**
- Coordination communautaire:

- Collaborer avec les partenaires locaux pour soutenir les aînés et assurer une continuité de services.
- Animer des réunions régulières avec les intervenants et les partenaires pour suivre les progrès du projet.
- Encadrement de l’équipe:

- Superviser et soutenir l’équipe d’intervenants, en veillant à leur bien-être et à leur développement professionnel.
- Coordonner un réseau de sentinelles pour identifier et accompagner les aînés vulnérables.
- Gestion de projet:

- Développer et utiliser des outils pour repérer les personnes âgées en difficulté.
- Contribuer à l’évaluation continue du projet et rédiger des rapports d’activité pour suivre son évolution.

- Un poste qui compte : Position stable et valorisante à plein temps de **35 heures** par semaine
- Rémunération compétitive à partir de 60 000$ par année en fonction de l’expérience
- Avantages sociaux : régime de retraite (contribution employeur de 4%/mois), 12 jours de congés personnels
- Vacances : 2 semaines dès l’embauche; 3 semaines après un an en poste
- Fériés supplémentaires durant les fêtes de fin d’année (organisme fermé)
- Date d’entrée en fonction **:septembre 2024**:

**Profil recherché**
- **Formation**_

Diplôme universitaire de 1er cycle en travail social, psychologie, gérontologie ou équivalent
- **Expérience**_

Minimum 3 ans d’expérience dans un poste similaire.
- **Compétences techniques**:_
- Intervention sociale : Très bonne connaissance des problématiques de santé mentale, de perte d’autonomie, d’exclusion sociale, de maltraitance et d’isolement social chez les personnes aînées.
- Analyse des besoins
- Gestion de projet
- Gestion d'équipe
- Maîtrise de la langue anglaise à l’oral
- Permis de conduire valide

- Aptitude à travailler en étroite collaboration avec différents partenaires et intervenants.
