Are you ready to inspire change and empower youth? We invite you to be part of our compassionate team at the Inner City Youth Program (ICYP) As a **Social Worker I, Youth Mental Health and Substance Use Case Coordinator**, you’ll play a vital role in supporting young people facing mental health and addiction challenges, working under the guidance of the Patient Resident Care Manager and the Professional Practice Leader of Social Work.
- ** Interprofessional Collaboration**: Work as part of an interprofessional team to provide clinical services to individuals, primarily youth and young adults, affected by mental health and addiction issues.**Client Coordination**: Coordinate the intake and transition of clients through the continuum of care. Assess clients, determine referral needs, and provide individual counseling, intensive case management, group therapy, and education.**Treatment Planning**: Formulate and implement treatment plans. Provide comprehensive case consultation and follow-up support on treatment decisions in collaboration with interdisciplinary team members.**Team Participation**: Participate in team and local committees, internal and external case conferences, and quality improvement activities.**Community Collaboration**: Collaborate with other community service providers to deliver education and training related to mental health and addiction.**Program Development**: Provide input into the development, evaluation, and delivery of the Inner City Youth Program.

What You Bring:
**Education and Experience Requirements**:

- Master's Degree in Social Work from an accredited university.
- Two (2) years of recent, related experience working with youth and young adults with mental health and substance use challenges in a community health setting, or an equivalent combination of education, training, and experience.
- Current full registration with the British Columbia College of Social Workers.

**Skills and Abilities**Knowledge of Addiction and Mental Health**: Demonstrated understanding of addiction issues, mental health challenges, and evidence-based treatment practices.**Assessment Proficiency**: Ability to conduct comprehensive psychosocial assessments using standardized tools for mental health and substance use.**Crisis Management**: Skilled in managing complex clinical and psychosocial crises within the youth and young adult population.**Interpersonal Communication**: Strong ability to establish rapport with diverse clients and communicate effectively with individuals and teams in a multicultural environment.**Organizational Skills**: Capable of organizing, prioritizing workloads, and developing educational resources and support groups.**Knowledge of Relevant Legislation**: Comprehensive understanding of relevant Acts, professional practice guidelines, and community resources for effective referrals and consultations.

**Your day to day**:
**Therapeutic Counseling**: Provides therapeutic clinical counseling services to clients, families, and groups by exploring personal concerns and treatment objectives, facilitating changes in coping styles, and coordinating services with interdisciplinary team members and community partners.**Psychosocial Assessments**: Conducts comprehensive social work psychosocial assessments through client interviews and data gathering regarding family, environment, culture, and financial status to enhance coping abilities.**Crisis Intervention and Support**: Delivers treatment services, including crisis intervention and education, by assessing situations, exploring emotional responses, identifying goals, and teaching coping strategies.**Care Options Development**: Assesses client needs and available services, develops care options with client
