Under the general direction of the Family Support Coordinator, the **Family Support Worker**: provides in-home assistance for families with children with special needs;**_ _**participates in the development of in-home intervention strategies; participates in the development of Annual Service Plans; assists the child in developing more independence, self-help and personal living skills; participates in the preparation of reports and other documents; participates as a Children’s Services team member.

**TYPICAL DUTIES AND RESPONSIBILITIES**:
Under the general direction of the Family Support Coordinator, the incumbent in this position will be required to:
**_Provide in-home assistance for families with children with special needs by:_**
- establishing rapport with the child and the parents/guardians
- participating in the identification of the child’s needs, strengths, and areas for growth and development
- providing the family with information, training and skill development
- providing the family with in-home parental relief, extraordinary day care, community aide, behavioural aide, developmental aide, personal care, etc.
- assisting the parents in involving the child in community activities
- being available to the parents for guidance, support, and assistance
- monitoring the progress of the child in relation to the Annual Service Plan
- monitoring the child in all areas of health and wellness
- providing the child and family with supports during times of crisis

Participate in the development of Annual Service Plans(ASP) by:_**
- promoting a positive relationship with the child and the child’s family and/or guardian
- gathering information on past history, family dynamics, the child’s likes/dislikes, strengths, etc.
- orienting the parents to the components of the ASP
- ensuring child’s personal choices and wishes are identified and respected
- clarifying/redefining responsibilities, roles, and expectations of the parents and of the child
- monitoring and evaluating the effectiveness of the ASP and suggesting modification as required
- participate in the completion of, and regular review, of the Workplace & Individual Hazard review(s)

Assist the child in developing more independence, self-help and living skills by:_**
- participating in the identification of each child’s needs in the area of personal growth and development
- participating in the development and implementation of goals for each child
- supporting the child and family in becoming more aware of and involved in the community
- working with the parents in the child’s development of socialization, safety and community
- providing constant supervision of the child
- monitoring the child’s progress in life style development according to Annual Service Plan goals and needs

Participate in the preparation of reports and other documents by:_**
- planning weekly activities and skill development ideas
- complete daily activity records and monthly reports
- monitoring the completion and accuracy of these reports
- maintaining a child binder in the home, ensuring it is up to date and current

Participates as a Children’s Services team member by:_**
- attending all team and staff meetings
- role modelling and promoting a positive environment for the child as per the program objectives
- demonstrating flexibility in job responsibilities when it is in the best interest of the child and/or parent/guardian
- fully participating, with a positive attitude, in the child’s activities by coming prepared in the manner required for community, social and/or other activities
- adhering on a continuous basis to the Code of Ethics and the Oath of Confidentiality

To be effective in the performance of the position described above, an individual would require the following:
**KNOWLEDGE**:

- Demonstrates knowledge and understanding of the values of **citizenship, choice, support, and equality of opportunity**, as defined and adhered to by the Parkland Community Living and Support Society
- Demonstrates knowledge and understanding of services as identified in the Canadian Accreditation Council of Human Services (CAC) standards
- Working knowledge of behaviour modification as practised by Parkland C.L.A.S.S.
- Working knowledge of crisis prevention and intervention
- General knowledge of the community amenities, social activities and opportunities
- Knowledge of children

**SKILLS**:

- Demonstrated ability to work with parents and children of Parkland C.L.A.S.S.
- Demonstrated ability to maintain professionalism in a family home, including the ability to identify and respect professional boundaries
- Good verbal and written communication skills
- Good housekeeping and home safety skills
- High degree of skill in motivation and interpersonal relationships
- Skill in defusing tense and sometimes dangerous situations
- Skill in providing a climate where parents and/or children will be motivated to take app
