The Greenstone Family Health Team provides comprehensive, collaborative, primary health care to the Northern Ontario Municipality of Greenstone. This exciting work requires caring, creative, and enthusiastic health care professionals. We are currently seeking a Social Worker to fill a 12-month contract.

**Position Summary**:
As vital a member of the interdisciplinary team, the Social Worker shall advocate for the best interest of the client and will operate within his/her full scope of practice. The Social Worker will work collaboratively with other health professionals and service providers to deliver client centred, coordinated care. The Social Worker is expected to perform duties within the guidelines set out by the Ontario College of Social Workers, and is responsible for assessment, treatment planning, system navigation, care coordination and psychotherapy/therapeutic counselling.

**Qualifications**:

- Master’s or Bachelor of Social Work Degree from an accredited university
- Registered member in good standing with the Ontario College of Certified Social Workers and Social Service Workers and the Canadian Association of Social Workers
- Experience in counselling/psychotherapy is required
- Excellent interpersonal, interviewing, treatment planning and system navigation skills
- Demonstrated knowledge of relevant legislation
- Bilingualism considered an asset

**Job Types**: Full-time, Fixed term contract
Contract length: 12 months

- Geraldton, ON P0T 1A0: reliably commute or plan to relocate before starting work (required)
