**ROLES AND RESPONSIBILITIES**

**PATIENT SAFETY**

Every employee's responsibility is to ensure that the hospital's patient safety goal continues to be the centrepiece of our quality and risk management program and that every patient is treated within a safe environment. To ensure compliance with this goal, each employee must:

- Find, report, and prevent incidents/near misses or adverse effects
- Communicate/report areas of concern immediately to your Manager
- Complete a Near Miss Form or Incident Report to communicate or report incidents or near misses

**SUMMARY OF POSITION**:
As a member of the health care team, the Social Worker provides direct patient care through the provision of psychosocial screening and assessment; individual and family counselling and treatment; palliative and bereavement counselling; liaison and referral to community resources; support with child welfare issues; support, advocacy and education.

**NATURE AND SCOPE OF WORK**

**SOCIAL SERVICE**
- Responsible in the performance of a therapeutic role including strong assessment, counselling, case management and advocacy skills
- Responsible for knowledge of the Health Care Consent Act, the Substitute Decisions Act and other related legislation
- Strong interpersonal, communication, teamwork, consultation and teaching skills
- Knowledge of community resources and referral processes
- Proven ability to work in multiple settings, and to organize, prioritize and problem solve effectively
- Commitment to ongoing learning, and involvement in committees and projects as required
- In depth understanding of various patient programs
- Advocate for patients and families, providing support and other options in community as needed.
- Collaborate in partnership with other external and reginal partnerships to support families/patients in hospital and within the community.
- Participate in various projects or research initiatives to support the implementation of evidenced-based care.

**HEALTH AND SAFETY**
- Responsible to be fully knowledgeable of responsibilities under the Occupational Health and Safety Act and Regulations and the hospital’s related policies and procedures and to work in compliance with those provisions in order to create and maintain an environment which is conducive to health and safety
- Work in a safe manner at all times, reporting hazards or incidents as soon as possible
- Ensure that employees work in compliance with established safe work practices, policies and procedures and attend hospital Health and Safety in-services when provided

**QUALIFICATIONS**
- Master’s Degree in social work with 3-5 years of clinical experience
- Current certification and in good standing with the Ontario College of Social Workers and Social Service Workers
- Strict confidentiality of all patients and hospital related information

**Salary**: $44.38-$55.39 per hour

Expected hours: 37.5 per week
