**The Community Disability Services Practitioner (CDSP) is an experienced disability worker who gives direct support to individuals with disabilities and takes part in discovery, assessment, quality of life planning and delivery.**

**Role and Responsibilities**
- Help the individual and support network develop and document the individualized quality of life plans to guide services related to personal development, community participation, behaviour management and/or personal care. Follow the individual’s and/or family’s desired direction to support their personal goals and choices as much as possible.
- Help carry out quality of life plans in more complex situations, without having the responsibility and power of a supervisor and make communication easy within the group.
- Help and encourage individuals to use their rights and take responsibility to make life choices and participate actively in meaningful and respectful relationships that fit with and support their goals and desires.
- Be aware of the individual’s physical, social and recreational environments, acting to minimize risks and maximize participation and inclusion and, when appropriate, asking for help from more experienced professionals.
- Be aware of and follow policies, practices, and operational protocols. This includes knowing and following ethical standards and guidelines, as well as medical protocols and standing orders for medication and other remedies.
- Help and encourage individuals to advocate on their own behalf, when required to have their needs met and, when appropriate, support the individual’s desires, goals and life choices by interacting and advocating with the individual’s social network.
- Help and encourage individuals to meet their own goals by developing life skills in order to enhance confidence, competence and participation in their social network (community, social, recreational, etc.). This includes aspects of personal development as needed
- Provide direct care to individuals with high needs and/or medically fragile individuals, as required.
- Find and develop employment, recreational and other social interaction opportunities for individuals in order to create a wider range of options and natural supports within the community.
- Be a social, recreational and/or employment role model and coach to help the individual participate and contribute effectively within the community and/or in an employment or volunteer setting. This includes being a role model and helping employers and others within the social network.
- Help employers and others in the individual’s social network create and maintain a safe and welcoming environment for participation.
- Create clear, objective records including the completion of required reports and other documentation.
- Communicate in a professional, timely, accurate and respectful way with individuals, families, guardians, staff and community members.

**Outcome Measures**
- Provide direction to CDSW and provide insight where applicable.
- Participate in the development and carrying out of the Quality of Life Plan and
- demonstrate how the individual’s goals relate to the domains and pillars of quality of life measures, basic human needs and social determinants of health.
- Participate and provide direction in Human Centered Design.
- Report and complete incident report documentation.
- Attend, participate, and when requested lead required meetings.
- Complete and maintain certification in mandatory training, demonstrating proficiency of
- skills in practice.
- Contribute in the assessment and demonstrate knowledge/proficiency of support
- procedures (Behaviors of Concern, Quality of Life Plan, Planned Positive and Restrictive Procedures as needed).
- Accurate completion of timesheet and mileage expense(s).
- Ability to demonstrate proficiency in Information Technology (IT) tools including the
- Read/complete daily documentation in My Compass.
- Ability to demonstrate how to serve as an ambassador of the organization actioning community involvement, recognizing individuals’ abilities, strengths and interests, demonstrating respect, courage and integrity.

**Job Types**: Full-time, Permanent
