- ** PURPOSE STATEMENT**

The SOS (Safer Opioid Supply) Case Manager is accountable for the development and delivery of a range of harm reduction and supportive treatment modalities for patients with opioid use disorder seeking an alternative to traditional OAT, in a patient centered model of care.

**JOB SUMMARY**

The SOS Case Manager is an individual contributor that collaborates with members of the CATC interdisciplinary team to plan, implement, coordinate, and evaluate the appropriate strategies to support patient goals of harm reduction and/or treatment.

**KEY DUTIES AND RESPONSIBILITIES**

**Medical**:

- Collaborates with members of an interdisciplinary team to plan, implement, coordinate and evaluate the appropriate treatment plans to support the patient.
- Supervise urine drug screening, and testing using a dipstick.
- If appropriately trained, performs venipuncture, ECGs, and assesses other vitals as required.
- Complies with all policies, procedures and protocols governing controlled substances.
- Management of Patient active scripts including script cancellation as per Rx protocol.
- Works within programs' quality assurance guidelines.

**Patient Services**:

- Communicates with MRP, Pharmacy and Nurse and Program Manager around patient needs, change in status and all emergency situations**.**:

- Facilitates clients in OTN sessions and conducts CARE calls.
- Provides comprehensive program information to current and prospective patients and families
- Responds to general inquiries and makes appropriate referrals within CATC Healthcare Team.
- Makes referrals to appropriate services, resources, and organizations to support client goals
- System navigation within the broader mental health, addiction, and health care space
- The ability to work with client case load virtually and in person

**Administration**:

- Acts as first point of contact and provide general information to patients and the public relating to their questions, concerns and/or suggestions.
- Maintain a high-level of competency in Electrical Medical Records use including verification of health card, scheduling, information entry and assessment, data management, and document management.
- Update, maintain and ensure accuracy of patient information in the Electronic Medical Records.
- Schedule and confirm intake and patient appointments.
- Maintain a sanitary work site and sterilization of equipment, keep bi-daily records of cleaning - includes emptying of garbage, sweeping and mopping, vacuuming, cleaning of bathrooms, and wiping of all surfaces; follows universal pre-cautions while performing all duties.
- Acts in accordance with the all relevant legislation, policies, standards, procedures and protocols.

**KNOWLEDGE, SKILLS AND ABILITIES**
- Strong commitment to working with individuals living with addiction and mental health.
- Experience in case management and system navigation.
- Knowledge and understanding of the community resources and services that would benefit patients/program participants.
- Ability to handle confidential and/or sensitive information.
- Ability to establish therapeutic relationships with patients and maintain professional boundaries.
- Excellent verbal and written communication skills to liaise with diverse internal and external professionals.
- Excellent organizational, time management and prioritization skills.
- Ability to work within a team environment and collaborate as an active participant with others in assisting in the delivery of care to patients.
- Knowledge and/or experience working with diverse communities. Knowledge and/or experience with trauma-informed approach to working with PWUD

**EDUCATION, QUALIFICATIONS, AND TRAINING**
- Post Secondary degree or diploma in the field of social services, addictions & mental health, harm reduction or vulnerable populations
- Previous experience in case management/system navigation or with vulnerable populations with addiction and mental health is considered an asset.
- Previous experience with community outreach is considered an asset.
- Provision of a recent and acceptable Criminal Reference Check.
- Valid drivers license and ability to reliably travel between sites as the business requires.
