St. Michael's Hospital is a full service tertiary care, academic health sciences centre founded by the Sisters of St. Joseph in 1892 to care for the sick and poor of Toronto's inner city. As downtown Toronto's adult trauma centre, our hospital is a hub for neurosurgery, complex cardiac and cardiovascular care, diabetes and osteoporosis care, mínimally invasive surgery and care of disadvantaged populations. We are also one of the province's major resources of care for critically ill patients.

This position is a contract position until May 31, 2018 with possible extension. Within each Sub Region the Primary Care Clinical Lead (PCCL) will have oversight responsibilities for Sub Region activities.

It must be noted that the deliverables and responsibilities may evolve to match provincial direction on the Primary Care Strategy.

The Primary Care Project Manager supports the development and implementation of the TC LHIN Primary Care strategy and assist the Primary Care Clinical Lead (PCCL) by organizing local engagement and sub-committee meetings, developing and analyzing sub-region specific data and providing data reports on performance and change activities.

DUTIES & RESPONSIBILITIES:

- Supports the Primary and Community Care Committee (PCCC), subcommittees and working groups by developing drafts of meeting materials and presentations, presenting at the meetings, and following up on issues and responses, as required.
- Supports the PCCL on all aspects of the primary care initiatives including support to the implementation of the TC LHIN's priority projects at the local level.
- Monitors performance and reports on activity; develops and analyzes data from various sources to determine performance and work plan results; produce dashboards /reports indicating performance and work plan activity; engage partners to continuously identify areas for improvement resulting in improved patient outcomes.
- Provides support to primary care providers on adoption of TC LHIN's approved initiatives; communicates and influences local partners to adopt changes andprovide expertise, consultation and support in implementation and evaluation ofinitiatives.
- Provides project management expertise by developing and drafting work plans; identify budget and resources; establish key steps and critical paths, develop project methods; manage priorities, monitor and report on progress, and resolving issues and conflicts.
- Organize and conduct stakeholder and health service provider engagements; facilitate meetings, prepare materials and support PCCL, as required.
- Co-ordinate the development and monitoring of priority areas and related projects, track expenses and prepare quarterly and ad hoc reports, as required. Working closely with the Primary Care Clinical Lead and the Primary Care Sub-Region Manager to determine the appropriate allocation of funding for projects and initiatives.
- Prepares timely, accurate and comprehensive business cases, position papers, briefing notes, project updates, promotional materials, internal/external correspondence and other executive documentation related to project planning, management and reporting.
- Support other TC LHIN and local sub-region initiatives including health links initiatives, care coordination model, meeting planning and any support required by the PCCL.

QUALIFICATIONS:

- Master's Degree in health or business administration coupled with 5 plus years related management experience or a combination of equivalent education and experience
- Excellent understanding of the Ontario health care system and primary care delivery models, and solid understanding of local health issues, priorities and needs.
- Demonstrated experience and ability to build positive working relationships with local health service providers and to influence decision making and adoption of change initiatives.
- Project management expertise with large scale change initiatives and proven ability to support change
- Excellent written and verbal communication skills and the ability to clearly convey information and ideas through a variety of media to individuals or groups in a manner that engages the audience and helps them understand and retain the message
- Strong planning and organizational skills; and the ability to facilitate meetings
- Able to understand and manage priorities with mínimal supervision
- Strong analytical, problem solving and decision making skills
- Experience in data analytics, epidemiology and information synthesis; knowledge of health care reporting requirements and funding methodologies, knowledge/familiarity with various databases used to capture information in primary care sector.
