SHIP is currently looking for a **Social Worker** who will be responsible for working with the clients as apart of our Assertive Community Treatment Team (ACTT). The Social Worker will provide supports for individuals experiencing mental health and/or problematic substance abuse challenges and are at risk of homelessness.

**Duties and Responsibilities**:

- Counsel and educate families regarding severe and persistent mental illness
- Interview individuals, families and significant others to assess clients’ social, emotional, physical and mental impairments, as well as financial needs
- Assist clients in obtaining appropriate community resources for financial support, such as ODSP, Ontario Works, supportive housing, etc.
- Lead group counselling sessions and plan psychosocial programming
- Obtain a thorough social history on clients and forward relevant concerns to the attention of all team members
- Provide individualized psychoeducation for families about illness and the role of the family and other significant people in the therapeutic process
- Support client self-determination in relation to legal matters (charges, diversion, probation, CTOs, SDMs, PGT- guardianship, POA, Mental Health Advance Directives and PHIPA).
- Share shift work including weekends and on call duties with other ACTT Staff is also required
- Perform additional duties as required/delegated

**Qualifications**:

- Bachelor’s Degree in Social Work or equivalent
- Professional Registration with OCSWSSW is required
- 3+ years’ experience with adults living with serious mental illness
- Experience with assessments, risk management, and crisis de-escalation
- Access to a reliable vehicle and valid Drivers License is required
- Able to acquire cleared Vulnerable Sector Police Records Check

**Working Conditions and Physical Requirements**:

- Day/Morning/Evening shifts (Monday - Friday) with the ability to work outside these hours where required
