**AREA OF RESPONSIBILITY**: To ensure the development and provision of culturally appropriate community support services through a variety of program formats specific to the needs of urban Indigenous seniors/elders, people with disabilities, the chronically ill, and those with special needs.

**SCOPE**:
The Lifelong Care program services chronically ill, cognitively impaired, physically disabled, frail, and elderly urban Indigenous people of all ages. The program provides transportation services; opportunities for socialization; service navigation and advocacy; and emotional and spiritual support to promote independent living and a good quality of life.

**KEY JOB FUNCTIONS**:

- Ensures Indigenous people have access to long-term care programs and services.
- Develop support services based on community-identified needs such as meal

programs, transportation, visiting, caregiver support, home maintenance and repairs, which includes a strengths-based action plan for each individual direct-service

user.
- Promotes awareness and ensures access to the program through a variety of

outreach efforts and promotional activities.
- Promotes the development of protocols and referral agreements as appropriate to

meet the needs and outcomes of program participants.
- Conducts ongoing community consultations and needs assessments related to

community-based services and long-term care needs.
- Accurately reports and ensures all program objectives are being met in accordance

with agreements.
- Outreaches and engages community partners and stakeholders as it pertains to the

needs of the direct-service users served and the program deliverables.
- Facilitates a strong community support system via respite care that will enable

families to provide mutual support to one another such as in support groups and healing circles.
- Participate on local long-term care community committees and boards to ensure

Indigenous representation and advocacy.
- Provides for the development of a comprehensive volunteer support circle.
- Ensures access to medical and Indigenous language translation services as required.
- Ensures all service activities utilize culture throughout all junctures of planning and

delivery.
- Utilizes quarterly reporting tools and database (if applicable) to input all data as required.

**Qualifications**
- Post-secondary degree and/ or Diploma in Indigenous studies, Health Studies, Social Work, or a combination of the equivalent and relevant education.
- Experience working within an Indigenous organization or community is an asset.
- Knowledge of Indigenous and mainstream supports and services.
- Strong knowledge and awareness of Indigenous culture and history, with particular emphasis on the impacts of trauma, family violence, and community outreach.
- A clear criminal reference check (CPIC) and Vulnerable Sector Clearance will be a condition of employment.

**Salary**: $46,000.00 per hour

Expected hours: 40 per week
