**KEY FUNCTION**:
Provide crisis assessments in hospital and community with police partners for clients with mental health addictions concerns within the policies, philosophy and objectives of the Mental Health and Addictions Program. Provide screening function for the Manitoulin site.

**REPORTING**:
Under the general direction of the Clinical Manager, Counselling and Treatment and Rural Programs.

**DUTIES**:

- Complete face to face crisis assessments for clients presenting in the Little Current and Mindemoya Emergency sites.
- Respond to referrals from police partners and attend mobile crisis visits with police as required.
- Perform community outreach as required.
- Provide crisis counselling via telephone.
- Complete intake assessments for incoming referrals.
- Develop and implement intervention plans based on best practice within a team approach to minimize or eliminate identified impacts and problems.
- Provide ongoing crisis management for clients waiting for treatment.
- Liaise with Emergency Department staff, police partners, family physicians and community stakeholders.
- Document and chart assessments, intervention plans, progress notes and outcomes in a timely manner.
- Assist and act as a resource for client and family education.
- Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.
- Provide guidance, training and orientation for new staff and students.
- Work collaboratively with community providers to provide wrap around care for complex clients.
- Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a four (4) year Bachelor’s Degree from an accredited university.
- Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
- Certification in Non-Violent Crisis Intervention (NVCI) is required.
- Certification/training in Crisis Intervention skills is preferred.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- Minimum of two (2) years full-time equivalent experience (within the last 5 years) working in community mental health or addiction settings.
- Minimum of two (2) years full-time equivalent experience working on an adult inpatient psychiatric unit or Crisis Intervention Program.
- Experience providing mental health and addiction assessments with recommendations and in facilitating the development of treatment goals with clients.

**KNOWLEDGE/SKILLS/ABILITIES**:

- Demonstrated knowledge of the Diagnostic Statistical Manual.
- Demonstrated knowledge of evidence based practices of recovery principles.
- Demonstrated knowledge of community resources.
- Demonstrated knowledge of the Mental Health Act and other relevant legislation.
- Demonstrated knowledge of Manitoulin Island community resources.
- Demonstrated excellent judgment and proven analytical skills.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Strong organizational skills with the ability to prioritize workload.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.

**PERSONAL SUITABILITY**:

- Successful criminal reference check is required for vulnerable sector.
- Ability to use tact and discretion in dealing with health care providers and patients.
