**Registry Assessor**

**1 Year Contract Full-Time (35hours/week) with possibility of extension**

The Alzheimer Society of Durham Region (ASDR) is a community support service organization that provides programs and services to people living with dementia and their care partners in Durham Region. We work closely with other service providers to ensure quality care and support for our clients to achieve our mission to improve the quality of life of people living with Alzheimer’s disease or a related dementia and their care partners.

Under the direction of the Director of Clinical Services, the Registry Assessor is responsible for the planning, implementation, administration, and evaluation of the Ontario Brain Institute (OBI) Screening Project throughout Durham Region. This position will primarily be responsible for conducting comprehensive assessments inclusive of cognitive testing in partnership with the Ontario Brain Institute (OBI) to increase cognitive testing accessibility and the development and implementation of a dementia registry. Further duties may be required from time to time to support the needs of the organization.

**What you will be doing**
- Maintains an advanced level of knowledge of Alzheimer’s disease and other dementias, including clinical manifestations, behaviours, current care practices, treatment options, available community resources, and all relevant legislation.
- Gathers information, conducts, or reviews relevant assessments and meets with clients to identify current and future needs, goals and level of risk.
- Administers cognitive assessments including, but not limited to the Montreal Cognitive Assessment (MoCA), Folstein’s Mini Mental State Examination (MMSE), and the Clock Draw Test (CDT).
- Establishes appropriate intervention plans to meet bio/psycho/social needs using a person/family-centered approach with internal and external referrals.
- Works collaboratively with AS staff to proactively facilitate linkages, communication, information exchange and coordination between clients and service providers, as appropriate.
- Monitors and provides proactive follow-up for clients and care partners within scope of role to ensure ongoing collaboration across services/providers and to identify opportunities for new or emerging care options to meet changing needs and to address service/support gaps.
- Uploads and/or provides assessment results to relevant physician EMRs, databases, by fax, and the registry, as required.
- Maintains confidential, accurate and current client records, including complete and thorough documentation for each client contact, in compliance with relevant privacy legislation and in accordance with professional standards and internal policies.
- Ensures that client consents, privacy, and confidentiality are maintained in compliance with legislation, professional standards/regulations and internal policies.
- Leverages and maintains positive working relationships with stakeholders, such as primary care providers, health care professionals, health, and community support service providers (e.g., hospitals, primary care) as required.
- Engages in regular outreach and proactive communication with stakeholders including sharing of assessment results and clinical findings and observations.
- Collect, maintain and report required quantitative and qualitative data to support province-wide and project-related monitoring, evaluation, and reporting.
- Participate in project implementation and outcome discussions as required.

**What you will bring along**
- Education: _
- Minimum bachelor’s degree in social work, gerontology, nursing, or other related health care discipline
- Registered health professional designation and master’s level education preferred
- Experience: _
- 3 to 5 years of client service experience in the health and/or social service sectors
- Experience administering cognitive assessments.
- Experience working directly with people living with Alzheimer’s disease or other dementias and their care partners
