We currently have an opportunity for a **Full-Time Social Worker (MSW) within Collaborative Practice Team.**

**Hours of Work**:4 weekdays and Saturdays, days with some evenings

You will work primarily in our outpatient interdisciplinary services. Responsibilities include but are not limited to:

- Provide clinical intervention to Pediatric Oncology Group of Ontario (POGO), Juvenile Diabetic, Cystic Fibrosis and other outpatient clinic youth, children and families.
- Provides support in Inpatient Childbirth, NICU, Inpatient Pediatrics, prenatal, antenatal and postpartum patients in specific circumstances
- Provide individual/family assessment, treatment recommendations, referral to community agencies and discharge planning for patients and families in response to the unique and complex needs of this population;
- Work effectively with members of the multidisciplinary team in establishing and monitoring processes, improving continuity of care for patients, and enhancing collaborative relationships with other care providers;
- Provide brief clinical interventions as well as crisis intervention as required, and facilitate referral to appropriate community agencies;
- Participate in service committees and quality improvement initiatives as required;
- Engages in professional development and continuing education;
- Assist with program development and evaluation as required;
- Participates in team building, service development and improvement activities striving to contribute to a healthy workplace environment.

**At Grand River Hospital, the Social Worker (MSW)**:

- Master of Social Work Degree
- Membership in the Ontario College of Social Workers and Social Services Workers
- Relevant clinical experience in the assessment and treatment of children, youth and families, to support the family-centered care philosophy;
- Demonstrated knowledge of best practice guidelines pertaining to the assessment and treatment of psychosocial and mental health challenges facing these populations;
- Working knowledge of the Mental Health Act, Community Treatment Orders, Health Care Consent Act, Child & Family Services Act and any other relevant legislation
- Demonstrated advanced skill in mental health assessment and treatment interventions;
- Demonstrated advanced skill in crisis management
- Proven ability to work independently and effectively with other health care providers and community services
- Proven ability to establish and maintain linkages with relevant formal and informal community services and resources
- Experience and skill in complex decision making processes, advancing best practice initiatives, and exploration of innovative models of care
- Excellent interpersonal, written and verbal communication, organization, and time management skills.
- Proven experience in self-directed learning and peer evaluation processes.
- Excellent attendance, punctuality, and work performance record
