Community Outreach Worker

Connecting with families and individuals identified as being in need of building support networks and/or emergency relief
- Work with populations at-risk of losing housing or already homeless
- Interview and assess clients to determine their needs, complete client intake and needs assessment
- Develop a plan of action with them to overcome some/all of their issues
- Take steps to promote self sufficiency amongst clients
- Collect and analyze data related to outreach efforts
- Educate individuals about available resources, programs, and services
- Provide support and assistance to individuals in accessing necessary services
- Collaborate with community organizations and stakeholders to enhance outreach efforts
- Maintain accurate records and documentation of outreach activities
- Make appropriate referrals to other agencies by connecting families and individuals to the appropriate resources, services and supports in their community in a supportive and educational role and non-intrusive way possible
- Maintain accurate client files and an up to date client list (including case notes, service plans etc.) in a secure and confidential fashion
- Maintain regular contact with the households on your client list, ensuring appropriate follow up is conducted and support plans are in place for families and individuals where necessary
- Spend a minimum of 75% of your time in direct service activities with families and individuals, and the remaining 25% will be in your home office.
- Maintain an updated and complete list of community resources available for the benefit of clients
- Ensure appropriate literature is available for distribution to clients via meetings and walk-ins
- Promote CRC programs and services to other agencies and the general public and provide education about the Community Outreach Program within the various communities of Wellington County and beyond.
- Participate in regularly scheduled Outreach Department Meetings, as well as Agency Staff meetings
- Collect and compile statistics and complete related reports as requested by the program manager
- Ability to work as a team player or an individual
- Clear Police Vulnerable Sector Check
- Ability to maintain confidentiality

**Experience**:

- Post Secondary degree or diploma in Social Services or related field
- Strong communication and interpersonal skills
- Ability to effectively educate and inform individuals from diverse backgrounds
- Proficient in administrative tasks such as data collection and record keeping and knowledge of Microsoft Teams considered an asset
- Knowledge of the Social Service System

**Salary**: $47,840.00 per year
