Position Summary

The Behavioral Support - Personal Support Worker (PSW) is part of an interdisciplinary team and is responsible for providing resident care to a group of residents under the direction of the RN and RPN. The Behavioral Support - Personal Support Worker (PSW) is expected to participate in the Behavioral Supports Ontario model, enhancing services and supports to older adults with complex and responsive behaviors associated with dementia, mental health, substance use and /or other neurological conditions and their caregivers. BSO care protocols, tools and any other approaches articulated in the LHIN BSO Action Plan will be utilized. These responsibilities shall be carried out in accordance with Universalcare mission, vision, values and leadership philosophy, policies and procedures, and applicable legislation and regulations.

The Behavioral Support - Personal Support Worker (PSW) is responsible for providing this care in a safe and competent manner within his or her scope of practice. The Behavioral Support - PSW is required to utilize the POC documentation and the plan of care. The Behavioral Support will link to system-wide resources for managing residents with complex behaviors by collaborating with service providers within the LTC sector and outside the LTC sector (e.g., Nurse-Led Outreach teams, Mobile Response Team, Geriatric Outreach Teams) for the development and implementation of a plan of care that addresses the resident’s responsive behaviors and stabilizes the resident in their care setting; Train and/ or advise LTC home and other health service providers (HSP) staff in behavioral service delivery and act as mentors and coaches to caregivers within a person’s circle of care. - Personal Support Worker (PSW), maintains communication and collaboration among LTC homes and other HSPs to facilitate partnerships, knowledge transfer, the spread of best practices, and otherwise enhance the behavioral support services available throughout the LHIN; and participate in relevant BSO Provincial Coordination Office (PCO) collaborative and other working groups.

RESPONSIBILITIES

In accordance with the established policies of Perth Community Care Centre, the Behavioral Support Worker
- Personal Support Worker (PSW) will:

- Participate in conducting behavioral assessments
- Implements and documents specific behavioral interventions
- Monitors and evaluates care as provided
- Coaches other direct care staff in the long-term care home as necessary
- Supports the Behavioral Support Ontario leads
- Updates and monitors behavior tracking forms/whiteboards
- Implements care plan recommendations
- Develops “A Day in The Life” snapshot with behavioral residents and families
- Participates in care conferences, including “circle of care” meetings for behavioral residents
- Assists in leading /supports BSO quality improvement initiatives
- Assists in the leading education and knowledge transfer to all staff on preventative behavior management
- Observe the general appearance of resident, seek direction in determining usual emotional, social, behavioral and physical pattern, recognize obvious change in usual pattern
- Communicate effectively as required for taking direction and performing job-related activities, both verbally and non-verbally.
- Provides cognitive, social, emotional support:

- Seeks direction in determining appropriateness of orientation or validation techniques
- Implements orientation or validation plan for residents
- Encourages resident’s indecision making process as per care plan
- Implements safety measures for the cognitively impaired resident
- Recognizes and reports safety hazards
- Implements simple measures to minimize aggressive behaviors as per care plan
- Observes and respects residents’ rights, dignity and privacy
- Develops supportive relationships with resident
