**Home Support Supervisor**
**Job Summary**:
The Home Support Supervisor is responsible for overseeing and managing home support services provided to individuals in their own homes. They ensure the delivery of high-quality care and support, coordinate the activities of home support workers, and maintain effective communication with clients, families, and other healthcare professionals. The Home Support Supervisor plays a crucial role in ensuring the well-being and safety of clients receiving care at home.
**Key Responsibilities**:
**1. Supervision and Team Management**:
Provide leadership, guidance, and supervision to home support workers.
Assign and delegate tasks to support workers, ensuring optimal coverage and workload distribution.
Conduct performance evaluations, provide feedback, and support professional development opportunities for staff.
**2. Client Assessment and Care Planning**:
Assess client needs and develop individualized care plans in collaboration with clients, their families, and healthcare professionals.
Regularly review and update care plans based on the changing needs and goals of clients.
Coordinate care services with other healthcare providers involved in the client's care, such as nurses, therapists, or social workers.
**3. Quality Assurance**:
Ensure compliance with established standards, policies, and regulations in the delivery of home support services.
Monitor the quality of care provided, conduct regular assessments, and implement improvement initiatives as needed.
Promote safety practices and infection control measures to maintain a safe environment for clients and staff.
**4. Staff Training and Education**:
Provide orientation and training to new home support workers, ensuring they have the necessary skills and knowledge to perform their duties effectively.
Conduct ongoing education and training sessions for support workers to enhance their skills and keep them updated on best practices.
Foster a culture of continuous learning and professional development among the team.
**5. Communication and Collaboration**:
Establish effective communication channels with clients, their families, and other healthcare professionals involved in the client's care.
Collaborate with other healthcare providers to ensure seamless coordination of services and address any client concerns or issues promptly.
Act as a liaison between clients, families, and the organization, addressing any questions, complaints, or feedback.
**6. Documentation and Reporting**:
Ensure accurate and timely documentation of client assessments, care plans, and services provided.
Maintain comprehensive records of client information, progress notes, and any changes in their condition or care requirements.
Prepare reports on home support services, including statistical data, quality indicators, and client outcomes.
**Qualifications**:
1. Bachelor's degree in Nursing or a related field. A master's degree is preferred.
2. Current professional license or registration as a Registered Nurse or relevant healthcare professional.
3. Vulnerable Sector Police Check and
4. TB Test - Medical Report (within 6 months)
5. Valid First AID and CPR level C
6. Proven experience in home care or community healthcare, with a minimum of 3-5 years in a supervisory or leadership role.
7. Strong knowledge of home support services, care planning, and best practices in home care.
8. Familiarity with relevant regulations, standards, and guidelines governing home care services.
9. Excellent leadership, organizational, and problem-solving skills.
10. Effective communication and interpersonal skills.
11. Proficiency in using electronic medical records (EMR) and other relevant software.
