**Temporary Patient Panel Manager/Receptionist**

**Kalyna County Primary Care Network (PCN)** is a joint venture between a group of family physicians from Vegreville, Viking, Vermilion, Killam, Tofield, Hardisty and Alberta Health Services (AHS) - who come together to implement a business plan to improve the quality of care and access to services for the people of our community.

Key Responsibilities:

- Works with all members of the clinical care team in a coordinated and collaborative effort to engage, encourage and support patient health enabling the physician to spend more time providing care to patients and less time performing “non-physician” activities.
- Utilizes health information technology to screen for preventable diseases including, but not limited to:

- cardiac health (BP, lipids, wt., BMI)
- cancer (pap, colonoscopy, mammogram)
- diabetes management (A1C, foot exam, BP, lipids, etc.)
- asthma and COPD management
- communicable diseases (immunizations)
- falls (bone density)
- lifestyle related (tobacco and alcohol use, obesity, exercise)
- Collaborates with Physician/NP on recommend preventative care and follow up actions such as referral to other interdisciplinary team members.

Education/Experience:

- Medical Terminology certificate or related experience.
- Medical Office Assistant or related experience.
- Experience in a healthcare setting.
- Experience using an Electronic Medical Record (EMR).
- Quality improvement experience
- Excellent computer and IT skills.
- Demonstrated and effective communication skills
- Ability to multitask
- Willingness to work in a dynamic environment that requires flexibility and balancing of competing priorities
- Valid driver’s license
- Grade 12 Graduate
- administrative assistant: 1 year (preferred)
- receptionist: 1 year (preferred)

Part-time hours: 30 per week

Expected start date: 2023-01-9
