Community Navigator

We are seeking a dedicated and compassionate **Community Navigator **to join the Ponoka FCSS team and assist us in serving the communities of Ponoka and Ponoka County. This is a new position t that will work closely with a multi-sectorial team. The position has been made possible through grant funding and has capacity to become a permanent position.

As a Community Navigator, you will play a vital role in connecting youth, young and middle adults and vulnerable populations with the resources and support they need to thrive. Your primary responsibility will be to assess needs, provide information and referrals, and guide them through the process of accessing community services.

**You will be tasked with**
- Conducting assessments to identify client and community needs and goals
- Providing information and referrals to community resources, such as healthcare, housing, employment, education, and social services
- Assist individuals in navigating complex systems and processes to access necessary services
- Develop and maintain relationships with community organizations and service providers
- Advocate for individuals and ensure they receive appropriate support
- Collaborate with interdisciplinary teams
- Monitor progress and provide ongoing support and guidance

**Education and Experience Requirements**:

- Diploma in social work, psychology, sociology, or a related field
- Proven experience working in a similar role, preferably in a community-based setting
- Strong knowledge of local community resources and services
- Excellent communication skills, both verbal and written
- Empathetic and compassionate approach to working with individuals from diverse backgrounds
- Ability to establish rapport and build trusting relationships
- Strong organizational skills and attention to detail
- Proficient computer skills

**Job Type**: Part-time

Pay: $34.00-$37.00 per hour

Expected hours: 20 per week
