**Position Summary**:
The Youth Empowerment Society is a non-profit community service agency offering support and services to youth and their families.

The Supported Independent Living Support Worker is responsible for the planning, coordination and implementation of the day-to-day operations for the program. The Support Worker aims to provide the supports necessary to ensure the successful transition of youth and youth adults to independence by preparing them to secure and maintain an independent living situation and to connect or reconnect these youth with the education system or employment market and with community resources.

The program will assist youth and young adults ages 15 -23 in the parental responsibility of the Ministry of Children and Family Development, with continuing care status or receiving services under section 12 of the Child, Family and Community Service Act, Youth Agreements or Agreements with Young Adults. Additionally, the program will assist youth not in care as they work towards securing housing and transitioning to independence.

**Duties and Responsibilities**:
To perform the job successfully the Supported Independent Living Support Worker must be able to consistently perform each essential duty adequately. Other duties may be assigned.

Youth/Family Engagement and Service Planning
- Works with youth, MCFD and families when appropriate, to encourage and facilitate the participation of individuals in the program to ensure that individual needs are being met.
- Facilitates referrals to appropriate community services, including identified medical, mental health, addictions, legal and family services.
- Develops and implements mentoring and intervention programs to assist MCFD and youth in determining goals and achieving the objectives developed.
- Assists each participant to develop an individual plan aimed to improve youths’ self reliance and to improve their chances of transiting to independence i.e. returning to school, obtaining and maintaining employment, improved family relations, relapse prevention, risk reduction, conflict resolution, appropriate leisure time practices, etc.
- Interviews youth, prepares case files and assesses problems and strengths. Provides intervention services collaboratively with other community programs.
- Assists participants both individually and collectively to develop personal management skills such as self awareness, problem solving, assertiveness, anger management, conflict resolution, group dynamics, dealing with authority, well being and nutrition, self esteem, financial management etc.
- Provides intervention and risk reduction for youth and their family support systems when required, especially around issues such as trauma, suicidal ideation, mental health and addictions.
- Forms mutually respectful partnerships with persons served and their family support systems to address any issues and problems they face through parent-teen mediation and conflict resolution.
- Provides direct one on one on-site supervision and support of youth accessing the program.
- Provides behavioural management skills, crisis response, intervention and risk reduction services to best support youth as well as create a safe and stable environment, especially around issues such as trauma, suicidal ideation, mental health and addictions.
- Teaching personal management skill development and leading group activities.
- Accepts the clients’ rights to self-determination and individuality and must not discriminate on the basis of race, ethnicity, language, religion, marital status, gender, sexual orientation, age, abilities, socio-economic status, political affiliations, or national ancestry.
- Participates in integrated case management conferences, as required
- Maintains a positive conviction about the capacity of people to grow and change.
- Sets appropriate boundaries, intervenes appropriately and maintains a helping role to meet the needs of the persons served or other family members.
- Facilitates group discussions and activities based on the programming schedule.
- Demonstrates a level of cultural sensitivity and understanding of the client population.
- Demonstrates a youth centred strength based approach.
- Recognizes persons with special needs and makes appropriate referrals.
- Acts as an advocate for youth and promotes empowerment for them to advocate on their own behalf.

Program Support and Administration
- Assists MCFD in providing outreach services and helping to reconnect youth to appropriate care giving systems in the continuum of care.
